摘　　要：目的:探讨左心室大小对行二尖瓣球囊形成术二尖瓣狭窄伴心房颤动患者的治疗效果及预后的影响。方法:对216例行二尖瓣球囊成形术患者实施随访,其中80例为二尖瓣狭窄伴心房颤动(心房颤动组),136例为窦性心律(窦性心律组),通过回访对影响早期事件发生的危险因素进行分析。结果:治疗前,两组左室舒张末期内径指数(LVEDDI)差异具有统计学意义(P<0.05);治疗后,两组在左房平均压、二尖瓣口面积、LVEDDI、再次行二尖瓣成形术、出现二尖瓣反流事件上差异均具有统计学意义(P<0.05),其中LVEDDI是心房颤动者独立危险因素,且LVEDDI>36 mm·m^(-2)较≤36 mm·m^(-2)更易发生早期危险事件。结论:二尖瓣球囊成形术对LVEDDI≤36 mm·m^(-2)的二尖瓣狭窄伴心房颤动具有较好疗效。Objective:To investigate the effect of left ventricular size on the clinical outcome and prognosis of the patients with mitral stenosis and atrial fibrillation undergoing mitral balloon angioplasty.Methods:216 patients with mitral valvuloplasty were followed up,80 cases of mitral stenosis with atrial fibrillation(atrial fibrillation group),136 cases of sinus rhythm(sinus rhythm group),the risk factors of early event were analyzed through the return visit.Results:Before treatment,the difference of left ventricular end-diastolic diameter index(LVEDDI)between the two groups was statistically significant(P<0.05);after treatment,the differences of the mean left atrial pressure,mitral orifice area,LVEDDI,mitral valve plasty again,mitral valve regurgitation between the two groups were statistically significant(P<0.05),and the LVEDDI was an independent risk factor for atrial fibrillation,and LVEDDI>36 mm·m-2 was more prone to early risk events than LVEDDI≤36 mm·m-2.Conclusion:Mitral balloon angioplasty for the mitral stenosis with atrial fibrillation has a good effect when the LVEDDI isn t more than 36 mm·m-2.
