摘　　要：目的探讨单导管标测指导下特发性右心室流出道(RVOT)频发室性早搏(VPC)、室速(VT)射频消融的有效性和安全性。方法回顾性分析本院2014年1月~2017年12月行单导管指导下射频消融治疗的特发性RVOT室早、室速患者共60例,男女比例2.1:1,平均年龄(42.3±16.3)岁,发病时平均年龄(38.1±16.1)岁。结果 60例中单导管激动顺序标测和起搏标测确定起源于间隔部34例,游离壁26例。射频消融术中及术后均无并发症发生,手术即刻成功率为100%,随访(17.4±3.6)个月,复发1例(复发率1.6%),总成功率为98.4%。结论特发性RVOT频发VPC、VT间隔部起源多于游离壁。单导管标测指导下射频消融右室流出道室性心律失常有效、安全。Objective To investigate the effectiveness and safety of single catheter mapping-guided radiofrequency ablation of ventricular premature beats(VPC)and ventricular tachycardia(VT)in idiopathic right ventricular outflow tract(RVOT).Methods A total of 60 patients with idiopathic RVOT in our hospital from January 2014 to December 2017 underwent radiofrequency catheter ablation were retrospectively analyzed,the proportion of men and women is 2.1:1,the average age(42.3±16.3)years old,the average age of onset(38.1±16.1)years old.Results In 60 cases,the single-vessel activation sequence mapping and pacing mapping were determined in 34 cases of the septum and 26 cases of the free wall.No complications occurred during and after radiofrequency ablation,the immediate success rate of the operation was 100%,followed up(17.4±3.6)months,1 cases recurred(1.6%),and the total success rate was 98.4%.Conclusion The frequent occurrence of VPC and VT interval in idiopathic RVOT is more than that of free wall.Radiofrequency ablation of right ventricular outflow tract ventricular arrhythmias under single catheter mapping is effective and safe.
