摘　　要：目的探讨烧伤患者伤残接受度及照顾者反应对患者生活质量的影响。方法 2016年9月至2017年5月,便利抽样法选择在福建省烧伤中心治疗的烧伤康复期患者216例及其照顾者216例为研究对象。采用一般资料调查表、伤残接受度量表修订版(acceptance of disability scale-revised,ADS-R)、精简烧伤健康量表(burn specific health scale-brief,BSHS-B)对患者进行调查,同时采用照顾者反应评估量表(caregiver reaction assessment,CRA)对照顾者进行调查。结果烧伤患者BSHS-B总分为(102.27±36.25)分。不同职业、经济状况、家庭居住地、烧伤原因及烧伤并发症的患者,其BSHS-B评分差异均有统计学意义(均P<0.05)。烧伤患者ADS-R总分为(70.37±19.04)分。照顾者CRA中健康问题维度为(11.39±3.26)分、自尊维度为(26.62±3.58)分、时间安排受打扰为(18.18±2.91)分、经济问题维度为(9.48±3.33)分、家庭支持缺乏维度为(13.32±3.70)分。烧伤患者ADS-R总分及各维度评分,照顾者CRA中健康问题、时间安排受打扰、经济问题、家庭支持缺乏维度评分,与患者BSHS-B总分及各维度评分均有相关性(均P<0.05)。有无烧伤并发症、烧伤患者ADS-R中扩大和控制维度评分及照顾者CRA中健康问题、家庭支持缺乏维度评分进入回归方程,可解释烧伤患者生活质量总变异的74.5%。结论烧伤患者生活质量处于中度偏低水平,医护人员应以提高烧伤患者伤残接受度、缓解照顾者照护压力为切入点,实施针对性的干预项目,以改善患者生活质量。Objective To explore the effect of acceptance of disability and caregiver reaction on the quality of life in patients with burns.Methods By convenience sampling,216 patients with burns treated in Burn Center of Fujian Province and their main caregivers were selected and surveyed with demographic questionnaire,acceptance of disability scale-revised(ADS-R),burn specific health scale-brief(BSHS-B)and caregiver reaction assessment(CRA).Results The total score of BSHS-B was(102.27±36.25)and there wereStatistically significant differences of BSHS-B in patients with different occupation,financial status,place to live,reasons of burn and complications(all P<0.05).The total average score of ADS-R was(70.37±19.04).The average score of caregivers’health problems,self esteem,disturbed time arrangement,financial problem,lack of family support in CRA was 11.39±3.26,26.62±3.58,18.18±2.91,9.48±3.33 and 13.32±3.70,respectively.The total score of ADS-R and scoresin its each dimension,the score of caregiver’s health problem,disturbed time arrangement,financial problem,lack of family support in CRA were correlated with patient’s total score of BSHS-B and score in each dimension(all P<0.05).The complications,total score of ADS-R and score of its each dimension in burn patients,score of caregiver’s health problem,lack of family support in CRA can explained the 74.5%of the total variance in patient’s quality of life.Conclusion The quality of life in burn patients is at the medium to low level.Medical and nursing staffs should implement target intervention by improving their acceptance of disability and relieving caregiver’s burden to improve patient’s quality of life.
