胸部肿物活检:超声与CT引导下的对比究

摘　　要：目的评价超声引导下穿刺活检在胸部占位性病变中的应用。方法选取2011年4月~2013年10月通过X线或CT发现的肺占位病变、胸壁病变和纵隔病变患者64例作为研究对象,经CT、MRI等影像学检查发现病变,分析对比两组取材成功率,病例阳性率及并发症。结果超声引导下活检取材成功率100%;组织学确诊阳性率为100%,并发症有少量气胸3%;咳血3%。CT引导下活检取材成功率100%;组织学确诊阳性率为84.4%,并发症有少量气胸2例;血胸3%,咳血6.3%。结论超声引导下胸部占位性病变穿刺活检,明确诊断率高,有较好的临床应用价值,但中心型肺占位或肺气干扰明显的占位须在CT引导下活检,两者能取长补短。Objective To evaluate ultrasound guided biopsy in space occupying lesions in the chest.Methods Methods from April 2011 to October 2013,64 cases of lung lesions,chest wall lesions and mediastinal lesions were detected by X-ray or CT,analysis and comparison of two groups based on the success rate,the positive rate of cases and complications.Results ultrasound guided biopsy success rate was 100%;the positive rate of histological diagnosis 100%,there are a few complications pneumothorax in 3%;hemoptysis 3%.CT guided biopsy success rate was 100%;the positive rate of histological diagnosis 84.4%,complications of pneumothorax in 2 cases;3%hemothorax,hemoptysis 6.3%.Conclusion ultrasound guided biopsy of chest lesions,the diagnosis rate is high,have a good clinical value,but pulmonary lesions or lung interference obvious mass should be guided biopsy in CT,both can learn from each other.
