盐酸奥布卡因凝胶在前列腺增生患者留置导尿中的临床应用

摘　　要：目的探讨盐酸奥布卡因凝胶在前列腺增生患者留置导尿中的临床应用价值。方法将2015年01月~2017年06月240例前列腺增生需留置导尿的患者,按照就诊先后顺序随机分为对照组(120例)和实验组(120例)。对照组采用石蜡油润滑导尿管前端,实验组采用盐酸奥布卡因凝胶润滑导尿管前端和尿道。分析比较两组患者置管一次成功率、置尿管时患者的疼痛程度、置管后尿道粘膜损伤程度、置管10 min后尿道刺激反应程度及置管后尿漏及尿路感染发生率等情况。结果实验组置管一次成功率为90.0%,对照组为56.7%,差异有统计学意义(x2=34.09,P<0.001);两组患者置管时疼痛程度(Z=-8.732,P<0.001)、置管后尿道粘膜损伤程度(x2=16.17,P<0.001)、置管10 min后尿道刺激反应程度(Z=-5.998,P<0.001)差异均有统计学意义;置管后尿漏及尿路感染发生率比较两组差异无统计学意义(x2=0.5 1 4,1.2 1 4,P>0.05)。结论前列腺增生患者留置导尿时,使用盐酸奥布卡因凝胶润滑尿管和尿道能明显减轻置管时的疼痛,降低患者置管后尿道粘膜损伤程度及置管后10 min尿道刺激反应程度,提高了一次性置管成功率,同时不增加漏尿及尿漏感染的发生率,值得临床广泛运用。Objective To investigate Oxybuprocaine Hydrochloride Gel in the clinical application of indwelling urethral catheterization in patients with benign prostatic hyperplasia.Methods From January 2015 to June 2017,240 cases of BPH with indwelling catheterization were randomly divided into the control group(120 cases)and the experimental group(120 cases)according to the order of visits.The control group using paraffin oil catheter tip,the experimental group used Oxybuprocaine Hydrochloride Gel lubricating catheter tip and urethra.Two groups were analyzed and compared with a success rate of catheterization and indwelling catheter when the degree of pain,patients after catheter,urethral mucosa after catheter 10 min urethral irritation reaction degree and the leakage of urine tube and the incidence of urinary tract infection etc.Result The experimental group a success rate of catheterization was 90%,56.7%in the control group,the difference was statistically significant(x2=34.09,P<0.001);two patients with catheter pain degree(Z=-8.732,P<0.001),the extent of urethral mucosal injury after tube(x2=16.17,P<0.001),the reaction degree of stimulation of urethral tube after 10 min(Z=-5.998 P<0.001),there were statistically significant differences;the infection rate was no significant difference between the two groups and urinary leakage of urine tube(x2=0.514,1.214,P>0.05).Conclusion Indwelling catheterization in patients with benign prostatic hyperplasia,and urethral catheter using Oxybuprocaine Hydrochloride Gel lubricating catheter can significantly reduce pain,reduce the degree of mucosal injury in patients with urethral tube and the reaction degree of stimulation of 10 min urethral tube,improve the success rate of catheterization,at the same time does not increase the incidence of leakage of urine and urinary leakage of infection.It is worthy of clinical application.
