三种填塞材料治疗尿毒症合并鼻腔出血疗效的观察

摘　　要：目的探讨尿毒症合并鼻出血患者凡士林纱条、膨胀海绵及鼻腔通气止血器三种鼻腔填塞材料填塞效果。方法 136例(180侧)尿毒症合并鼻出血患者随机分为随机分为凡士林纱条组、膨胀海绵止血组和鼻腔通气止血器组;选取填塞即刻、8小时、48小时观察头疼(鼻痛)及撤除填塞物后鼻腔出血及鼻内镜Lund-Kennedy黏膜形态评分,比较三种填塞材料的疗效。结果三组患者疼痛评分,即刻疼痛凡士林纱条组较其余两组明显,有统计学意义;8小时三组相当,无统计学意义;48小时鼻腔通止血器组疼痛较凡士林纱条组、膨胀海绵组轻微,差别有统计学意义。撤除填塞物后出血及Lund-Kennedy黏膜形态评分,鼻腔止血器组与其余两组相比有统计学意义。结论尿毒症合并鼻出血患者填塞止血,鼻腔通气止血器较凡士林纱条、膨胀海绵有优势,值得推广。Objective To investigate the effect of nasal packing materials of uremic patients with nasal bleeding Vaseline gauze,expansion sponge and nasal ventilation and hemostasis.Methods One hundred and thirtysix patients(180 sides)with uremia and epistaxis were randomly divided into Vaseline gauze group,expanded sponge hemostat group and nasal ventilator group.The headache was observed 8 hours and 48 hours),Nasal cavity bleeding and nasal endoscopic Lund-Kennedy mucosal morphous score were compared between the two kinds of packing materials.Results The pain scores of the three groups were significantly higher than those of the other two groups(P<0.05).There was no significant difference between the three groups in the 8 hours,the pain of the nasal cavity hemostasis group was significantly lower than that of the Vaseline gauze group The difference was statistically significant.Hemorrhage and Lund-Kennedy mucosal morphology were observed in the nasal cavity hemostasis group,compared with the other two groups.Conclusion Hemostasis of uremia patients with nasal hemorrhage,nasal ventilation hemostasis device than Vaseline gauze,expansion sponge has the advantage,it is worth promoting.
