窥视睾丸的常规手术和腹腔镜下手术疗效的临床比较

摘　　要：目的比较常规手术方式和腹腔镜下手术治疗儿童窥视睾丸的临床疗效、微创优势及病例选择。方法通过回顾2010年01月以来行睾丸下降固定术的隐睾患儿的资料,满足条件:(1)术前B超可明确探及睾丸的隐睾患儿。(2)在体格检查时发现患侧睾丸时可触及,时不可触及的隐睾患儿。共计39例,均为单侧,其中右侧为26例,左侧13例。就诊年龄1~10岁,平均年龄2.7岁。由于手术开展时间及方式的不同,分组为常规手术组和腹腔镜手术组。比较两组的手术时间、术中睾丸下降位置、围手术期并发症、术后随访并发症、患儿家属满意度。结果 39例手术均顺利完成,手术平均时间为53 min和68 min,常规手术组,有13例无法放置入阴囊中部及底部,而腹腔镜下通过腹膜后高位松解精索血管及输精管,全部能下降入阴囊中部及底部。随访>6个月,除常规手术组1例睾丸自阴囊上极回缩至外环口下方,阴囊褶皱明显,其余均无萎缩,无回缩。其中满意度调查,腹腔镜组为100%满意,而常规手术组为86.9%。结论窥视睾丸在腹腔镜下行睾丸下降固定术,不仅能一期完全把睾丸下降入阴囊底部,而且不破坏腹股沟管结构,具有手术创伤小,恢复快,并发症少,睾丸能完全下降入阴囊中部及底部,患侧腹股沟处无手术疤痕,更美观,家属满意度高,窥视睾丸在腹腔镜下行睾丸下降固定术比传统手术优势明显。Objective To compare the clinical curative effect,minimally invasive advantage and case selection of children's peeping testicles in routine operation and laparoscopy operation.Methods To review the data of the cryptorchidism after the year of 2010.①B ultrasound can determine the cryptorchidism.②A child with cryptorchidism physical examination can be touched or not beis touchedtesticles.39 cases,divied into laparoscopy operation group and routine operation group,compared operation time,location testicular descent,perioperative complications,and postoperative complications,follow-up families of children with satisfaction.Results All 39 cases were completed successfully and the average time was 53min and 68min.During the follow-up of>,6 months,1 patient of the routine operation group had testicular retraction,and the satisfaction degree of the laparoscopy operation group was higher than that of the routine operation group.Conclusions Peeping testicles descent and fixation under laparoscopy operation,not only can descent and fixation into the scrotum completely,and does not destroy the inguinal canal,a small operation trauma,fewer complications,small surgery scars,more beautiful,family satisfaction is high,and peeping testicles descent and fixation under laparoscopy operation obvious advantages than routine operation.
