化浊通痹合剂治疗老年糖尿病周围神经病变的研究

摘　　要：目的评价化浊通痹合剂治疗老年糖尿病周围神经病变的临床疗效及优势。方法选取我院收治的老年糖尿病合并周围神经病变患者200例分为治疗组及对照组各100例。两组均给予生活方式干预治疗+口服降糖药或皮下注射胰岛素的基础治疗,同时治疗组给予口服化浊通痹合剂,治疗8周,观察两组患者治疗前后中医证候疗效、血糖、血脂、Toronto临床评分及双下肢动脉彩色超声多普勒血流动力学情况。结果治疗组中医证候疗效评定结果总有效率为81.9%,明显优于对照组27.5%,差异有统计学意义(P<0.05);治疗组FPG、2 hPG水平较治疗前均明显下降,CHOL、TG、LDL水平较治疗前降低,HDL水平升高,差异有统计学意义(P<0.05),且均优于对照组,差异有统计学意义(P<0.05);两组治疗后Toronto临床评分各项分数及总积分较本组治疗前均有改善,但仅治疗组比较,差异有统计学意义(P<0.05);比较两组患者治疗前后双下肢动脉内径变化及血流峰值速度变化情况,治疗组双下肢胫前动脉、胫后动脉、足背动脉内径及对照组胫后动脉内径较治疗前增宽,差异有统计学意义(P<0.05),双下肢胫前动脉、胫后动脉及足背动脉血流峰值速度明显上升,上升程度均优于对照组,差异有统计学意义(P<0.05)。结论化浊通痹合剂治疗老年糖尿病周围神经病变临床疗效优势显著,能够有效延缓糖尿病靶器官损害,值得临床推广应用。Objective To evaluate the change chaotic tong bi mixture treatment the curative effect of senile diabetes peripheral neuropathy and advantages.Methods Random number table method was applied to 200 cases of patients with senile diabetic peripheral neuropathy patients were divided into treatment group and the control group(n=100).In the two groups were given lifestyle intervention treatment+oral medications or subcutaneous injection of insulin treatment,treatment group at the same time giving oral change chaotic tong bi mixture,the eight weeks of treatment,efficacy syndromes was observed before and after treatment in both groups,blood glucose,blood lipid,Toronto clinical scoring and double lower limbs artery color doppler ultrasound blood flow dynamics.Results The treatment group of TCM syndrome curative effect evaluation of the results the total effective rate was 81.9%,obviously better than the control group 27.5%(P<0.05);Treatment group(FPG,2 HPG level before treatment were significantly decreased,CHOL,TG,LDL level was reduced,the HDL levels,the difference had statistical significance(P<0.05),and are better than that of control group(P<0.05);Toronto clinical grading the two groups after treatment the score and total score a group before the treatment are improving,but the treatment group was statistically significant difference(P<0.05);Compare two groups of patients before and after treatment of double lower limbs artery diameter and blood flow peak velocity changes,the treatment group double pretibial lower limb artery,posterior tibial artery,posterior tibial artery diameter dorsalis pedis artery diameter and the control group was broadening,the difference was statistically significant(P<0.05),the double pretibial lower limb artery,posterior tibial artery,dorsalis pedis artery blood flow peak velocity obviously rise,rising degree were superior to control group,the difference was statistically significant(P<0.05).Conclusion Change chaotic tong bi mixture of senile diabetes peripheral neuropathy clinical curative
