水胶体和自粘性薄膜敷贴在预防PICC置管后局部皮炎的效果比较

摘　　要：目的比较水胶体和自粘性薄膜两种透明敷贴在预防PICC置管后局部皮炎的效果。方法选取我院2015年1月~2015年12月收治的恶性肿瘤需长期化疗并同意行PICC置管术患者240例作为研究对象,按随机对照盲法分为水胶体敷贴组和自粘性薄膜敷贴组各120例。两组均采用相同材质的PICC管,相同的穿刺和术后护理,观察PICC置管后皮炎发生率、因皮炎导致拔管比率,同时比较两组患者局部皮肤情况及舒适度。结果水胶体组最终完成研究117例,发生局部皮炎4例,发生率为3.42%,自粘性薄膜组最终完成研究115例,发生局部皮炎17例,发生率为14.78%,差异有统计学意义(P<0.05)。水胶体组4例皮炎患者中轻、中、重度的构成比例分别为75%、25%和0%,因皮炎导致拔管率为0%;自粘性薄膜组轻、中、重度的构成比例分别为23.53%、47.06%和29.41%,因皮炎导致拔管率为5.88%,差异有统计学意义(P<0.05)。水胶体组粘连发生率为7.69%,低于自粘性薄膜组的20.0%,差异有统计学意义(P<0.05);水胶体组无疼痛和舒适患者分别为85.47%、87.18%,高于自粘性薄膜组的70.43%、58.26%(P<0.05)。两组患者皮肤破损率比较,差异无统计学意义(P>0.05)。结论水胶体敷贴在预防PICC置管后局部皮炎发生方面效果优于自粘性薄膜敷贴,而且还可提高患者舒适度。Objective To compare the water gel and two transparent self-adhesive film sticking in the prevention of PICC catheter after the effect of local dermatitis.Selection.Methods From January 2015-December 2015 240 cases of malignant tumor should be long-term chemotherapy and agrees that the PICC line tube insertion.we patients as the research object,the scholastic ethics committee approval,patients signed the informed consent,according to randomized blinded divided into water colloid sticking group(120 cases)and self-adhesive film sticking group(120 cases).PICC tube of two groups all use the same material,same puncture and postoperative nursing,observation of PICC catheter after dermatitis incidence,as a result of dermatitis decannulation rate,at the same time to compare two groups of patients with local situation and comfort skin.Results Hydrocolloid group eventually complete the study of 117 cases occurred 4 cases,partial dermatitis incidence was 3.42%,the viscous film set of finishing the 115 cases,17 cases occurred local dermatitis,the incidence was 14.78%(P<0.05).Hydrocolloid groups of 4 patients with dermatitis,the composition of light,medium and heavy proportion are 75%,25%and 75%respectively,as a result of dermatitis decannulation rate was 0%;Self-adhesive film group composition of light,medium and heavy proportion is 23.53%,47.06%and 29.41%respectively,as a result of dermatitis decannulation rate was 5.88%(P<0.05).Water conglutination colloid group was 7.69%,less than 20.0%of the self-adhesive film group(P<0.05);Water gel group of patients with pain and comfort were 85.47%,87.18%,70.43%higher than that of self-adhesive film group,58.26%(P<0.05).The patient skin damage rate is similar between the two groups has no statistical significance(P>0.05).Conclusion Water colloid sticking in the local dermatitis occurs after PICC catheter prevention effect is better than that of self-adhesive film patches,but also can improve patient comfort.
