胆总管探查术后早期胆漏的综合治疗

摘　　要：目的探讨对症处理、营养支持、生长抑素在胆总管探查术后早期胆漏的治疗效果。方法 16例胆总管探查术后早期胆漏患者,在一般对症处理的基础上使用生长抑素并早期肠内营养支持治疗,观察治疗前后腹腔引流量、淋巴细胞计数、氮平衡、血清白蛋白和体重变化。结果治疗后患者腹腔引流液量即显著减少,差异有统计学意义(P<0.05);淋巴细胞计数、氮平衡增加、血清白蛋白、体重变化差异不显著,所有患者均治愈,未行手术治疗。结论对症处理、积极营养支持、给予生长抑素等的综合治疗,能够维持患者的营养状况及内环境稳定,促进漏口的恢复,有效地治疗胆总管探查术后早期胆漏。Objective To investigate the effect of symptomatic treatment,nutritional support and somatostatin in the treatment of bile leakage at the early stage after common bile duct exploration.Methods 16 patients with bile leakage after common bile duct exploration were treated with somatostatin and early enteral nutrition support on the basis of general symptomatic treatment,The volume of peritoneal flow,lymphocyte count,nitrogen balance,serum albumin and weight change were observed before and after treatment.Results After treatment,the amount of peritoneal drainage was decreased significantly(P<0.05),there was no significant difference in lymphocyte count,nitrogen balance,serum albumin and weight change,all patients were cured without surgical treatment.Conclusion The comprehensive treatment of symptomatic treatment,active nutrition support and somatostatin can maintain the nutritional status and internal environment stability of the patients,promote the recovery of the leakage,bile leakage early effective treatment after common bile duct exploration.
