鼻内镜下中鼻甲的处理对慢性鼻-鼻窦炎合并鼻息肉患者的影响分析

摘　　要：目的分析探讨鼻内镜下中鼻甲的处理对治疗慢性鼻-鼻窦炎合并鼻息肉患者的影响。方法选取2015年1月~2016年12月我院收治的36例行鼻内镜下功能性鼻窦手术对中鼻甲结构异常的处理,并随机分为中鼻甲成形术的保留组24例(38侧)。部分中鼻甲切除或全部切除术的切除组12例(20侧)。根据患者鼻塞、流脓涕、头疼、嗅觉障碍等主观症状及Lund—Kennedy内镜检查结果进行评价。结果所有患者通过6个月随访复查,保留组中鼻塞、流脓涕、头疼、嗅觉恢复等明显优于切除组,两组差异明显具有统计学意义(P<0.05)。结论鼻内镜下功能性鼻窦术在治疗慢性—鼻窦炎合并鼻息肉患者中对形态异常的中鼻甲进行保留式的处理,对鼻腔、鼻窦功能的恢复,改善鼻塞、头疼、流脓涕的症状有明显的疗效。Objective To analyze the effect of endoscopic treatment of nasal turbinate on patients with chronic rhinosinusitis with nasal polyps.Methods From January 2015 to December 2016,36 cases of functional endoscopic sinus surgery for middle turbinate structural abnormalities were retrospectively analyzed.They were randomly divided into 24 cases(38 sides)of middle turbinate plasty group.12 cases(20 sides)were excised in partial or total excision of middle turbinate.According to the evaluation of patients with nasal congestion,nasal,headache,and other symptoms and subjective olfactory disorders Lund Kennedy endoscopy results.Results All patients through 6 months of follow-up,the preserving group in nasal congestion,nasal,headache,olfactory recovery was significantly better than the resection group,the difference between the two groups was statistically significant(P<0.05).Conclusion Endoscopic sinus surgery in the treatment of functional treatment of retained abnormal morphology of middle turbinate in patients with chronic sinusitis with nasal polyps,on the function of nasal cavity and paranasal sinus recovery,has obvious curative effect to improve the nasal congestion,headache,nasal symptoms.
