摘 要:目的探讨去细胞肌基膜管桥接大鼠脊髓的修复效果。方法将32只成年雄性大鼠制成脊髓半横断损伤模型,一组脊髓损伤处植入肌基膜管,另外一组在植入肌基膜管后再用邻近椎旁肌带蒂转移覆盖,空白组不做处理。术后4周取材,光、电镜观察神经纤维的生长情况和移植区毛细血管的生成情况。结果带蒂肌瓣覆盖组的大鼠脊髓损伤处有较多的神经纤维通过脊髓断端,同时有毛细血管生成现象,血管从肌肉延伸到脊髓实质。电镜结果显示有大量神经轴突的生成,大鼠的继发性损伤在很大程度上有所降低。结论肌基膜管可显著促进损伤脊髓的神经纤维再生,带蒂肌瓣覆盖可促进损伤区血管生成。Objective To observe effects of chemically extracted acellular muscle graft in promoting axonal regeneration after spinal cord hemisection in adult rats.Methods Totally 32 male rats underwent spinal cord lateral hemisection.Treatment consisted of application of chemically extracted acellular muscle graft(CEAM),CEAM and covering pedicle fresh muscle into the lesion gap,which was left empty in lesion group.Rats in each group were killed 4 weeks after surgery.The subjects were observed,including the histological staining.Finally electron microscope was used to observe the changes of the axon and blood capillary.Results Many regenerating axons were noted to extend into the CEAM implanted lesion with covering pedicle fresh muscle.Dyeing experiment object showed pedicle muscle flap to cover the subjects had blood capillary growth of spinal cord injury,and the blood capillary from muscle extension to the spinal cord parenchyma.At the same time,the electron microscope results showed that the good located the place where nerve fibers generate.This method showed that the subjects of secondary damage reduced to a great extent.Conclusion CEAM can promote robust regrowth of axons in spinal rats.Covering pedicle fresh muscle into the lesion gap a can effectively strengthen the blood Supply in the ischemic area.
