食管壁内支气管囊肿的临床分析

摘　　要：目的分析总结食管壁内支气管囊肿的临床特征,以提高对本病的认识及诊治能力。方法通过万方数据库、中国知网中文文献数据库、维普中文数据库检索1976年1月~2016年7月报道食管壁内支气管囊肿的文献,结合四川大学华西医院(笔者进修医院)近收治的1例患者,回顾性分析食管壁内支气管囊肿的临床资料。结果本组1例,文献检索到52例,共53例。53例食管壁内支气管囊肿患者中,男性多于女性,好发于食管下段,且无典型的临床症状及体征,术前误诊率高。51例患者通过外科手术治疗,2例患者通过内镜黏膜下剥离摘除囊肿,术后病理示支气管囊肿。结论食管壁内支气管囊肿临床罕见,临床表现无特异性,容易误诊,最终多通过手术和病理得到确诊,考虑到囊肿有破裂和出血的风险,故手术治疗是必要的,并且临床手术治疗效果好。Objective To analyze and summarize the clinical characteristics of intramural esophageal bronchogenic cyst,and improve the understanding,diagnosis and treatment of this disease.Methods Literatures were searched for reports on intramural esophageal bronchogenic cyst between January 1976 and July 2016 by Wan fang database,China National Knowledge Internet(CNKI)and VIP database,and the clinical data of intramural esophageal bronchogenic cyst was retrospectively analyzed by combining the 1 patient treated in West China Hospital of Sichuan University(where the author received further education).Results The group of one patient,and 52 cases in the literature retrieval,a total of 53 patients.There are more males than females,which occurs mainly in the lower esophageal segment,without typical clinical symptoms and signs,and the preoperative misdiagnosis rate is high.51 patients were treated through surgical operations,2 patients with endoscopic submucosal dissection excision cyst.All postoperative pathologies suggested bronchogenic cyst.Conclusion Intramural esophageal bronchogenic cyst is rare in clinic and shows no specificity in clinical manifestations,resulting in easy misdiagnosis.most cases are finally confirmed by surgery and pathology.Considering the risks of cyst rupture and hemorrhage,surgical treatment is necessary and associated with optimal prognosis.
