天舒片治疗后循环缺血合并高血压患者的临床疗效及其对内皮细胞膜微粒表达的影响

摘　　要：目的观察天舒片治疗后循环缺血合并高血压患者的临床疗效,探讨其对内皮细胞膜微粒(EMPs)表达的影响。方法选取2015年1月—2017年1月西南医科大学附属医院神经内科收治的后循环缺血合并高血压患者100例,采用随机数字表法分为对照组和治疗组,每组50例。对照组患者给予常规治疗,治疗组患者在对照组基础上给予天舒片治疗;两组患者均连续治疗2周。比较两组患者临床疗效,治疗前后眩晕程度评定量表评分、椎-基底动脉平均血流速度、外周血EMPs表达情况;观察两组患者治疗期间不良反应发生情况。结果治疗组患者临床疗效优于对照组(P<0.05)。治疗前两组患者眩晕程度评定量表评分、椎-基底动脉平均血流速度比较,差异无统计学意义(P>0.05);治疗后治疗组患者眩晕程度评定量表评分低于对照组,椎-基底动脉平均血流速度快于对照组(P<0.05)。治疗前两组患者外周血CD_(31)^+/CD_(42)b^-微粒、CD_(144)^+微粒、CD_(62)E^+微粒分数比较,差异无统计学意义(P>0.05);治疗后治疗组患者外周血CD_(31)^+/CD_(42)b^-微粒、CD_(144)^+微粒分数低于对照组,而外周血CD_(62)E^+微粒分数高于对照组(P<0.05)。治疗期间治疗组患者出现2例轻微胃部不适。结论天舒片治疗后循环缺血合并高血压患者的临床疗效确切,可有效减轻患者眩晕程度,改善椎-基底动脉血流,调节EMPs的表达,且安全性较高。Objective To observe the clinical effect of Tianshu tablets in treating posterior circulation ischemia patients merged with hypertension,to investigate the impact on expression of endothelial microparticles(EMPs).Methods From January 2015 to January 2017,a total of posterior circulation ischemia patients merged with hypertension were selected in the Department of Neurology,the Affiliated Hospital of Southwest Medical University,and they were divided into control group and treatment group according to random number table,each of 50 cases.Patients in control group received conventional treatment,while patients in treatment group received Tianshu tablets based on conventional treatmet;both groups continuously treated for 2 weeks.Clinical effect,Vertigo Rating Scale score,average blood flow velocity of vertebrobasilar artery and peripheral blood EMPs expression before and after treatment were compared between the two groups,and incidence of adverse reactions was observed during treatment.Results Clinical effect in treatment group was statistically significantly better than that in control group(P<0.05).No statistically significant differences of Vertigo Rating Scale score or average blood flow velocity of vertebrobasilar artery was found between the two groups before treatment(P>0.05);after treatment,Vertigo Rating Scale score in treatment group was statistically significantly lower than that in control group,while average blood flow velocity of vertebrobasilar artery in treatment group was statistically significantly faster than that in control group(P<0.05).No statistically significant differences of peripheral blood CD31+/CD42b-microparticles percentage,CD144+microparticles percentage or CD62E+microparticles percentage was found between the two groups before treatment(P>0.05);after treatment,peripheral blood CD31+/CD42b-microparticles percentage and CD144+microparticles percentage in treatment group were statistically significantly lower than those in control group,while peripheral blood CD62E+microparticles perce
