拉贝洛尔联合硝酸甘油控制性降压在肩关节镜手术中的效应观察

摘　　要：目的:探究在肩关节镜手术控制性降压中,硝酸甘油、拉贝洛尔联合的应用价值。方法:从赣州市人民医院收治接受肩关节镜手术的患者中选取60例作为研究目标,依据手术日期的单双号对其实行随机分组:研究组(30例)和参照组(30例),分别通过硝酸甘油联合拉贝洛尔、硝酸甘油予以治疗,对2组患者的治疗成效进行对比。结果:参照组的术中输血量、出血量和硝酸甘油用量均显著多于研究组,且P<0.05;参照组的拉贝洛尔使用量显著低于研究组,且P<0.05。2组在T1、T3时点的血压比较,差异无统计学意义(P>0.05);T2时点参照组的血压明显高于研究组,P<0.05;2组T2时点的血压显著低于T1,P<0.05;2组T3时点的血压与T1比较,P>0.05。2组在T1、T3时点的心率比较,P>0.05;T2时点参照组的心率明显快于研究组,P<0.05;参照组T2时点的心率显著快于T1,P<0.05;研究组T2、T3时点的心率与T1比较,P>0.05;参照组T3时点的心率与T1比较,P>0.05。结论:在肩关节镜手术控制性降压中,硝酸甘油、拉贝洛尔联合使用,不但可以达到降压效果,还不会使患者的心率增快,有效地保障患者在控制性降压过程中的安全。Objective:To explore the application value of nitroglycerin and Labetalol in controlled hypotension under shoulder arthroscopy.Methods:Sixty patients with shoulder arthroscopy were selected as the research target.According to the single and even number of the operation date,the patients were randomly divided into study group(n=30)and control group(n=30).The two groups were treated with nitroglycerin combined with Labe and nitroglycerin respectively.The therapeutic effects of the two groups were compared.Results:The amount of intraoperative blood transfusion,blood loss and nitroglycerin in the reference group were significantly higher than those in the study group(P<0.05).There was no significant difference in blood pressure between the control group and the study group(P>0.05)at T1 or T3,and there was no change between T1 and T3 in the two groups(P>0.05).The blood pressure and heart rate was lower in study group than that in the reference group(P<0.05)at T2.The heart rate of the reference group at T2 time was significantly faster than that of the T1(P<0.05);and there was no difference between T1,T2 and T3 in the study group(P>0.05).And there was no difference between T1 and T3 in the reference group(P>0.05).Conclusion:The combined use of nitroglycerin and Labetalol in the controlled hypotension under shoulder arthroscopy can not only reduce the blood pressure,but also do not affect the heart rate of the patients,and ensure the safety of the patients during the controlled hypotension.
