多种实验室检测指标在结核患者中的诊断价值探讨

摘　　要：目的:探讨痰液涂片镜检、结核感染T细胞斑点试验(T-SPOT.TB)、血清结核抗体、血沉等四项检测对结核的诊断价值。方法:分别对97例结核患者及27例非结核患者分别进行液涂片镜检、T-SPOT.TB、血清结核抗体、血沉等检测。结果:除血沉外,结核患者液涂片镜检、T-SPOT.TB、血清结核抗体阳性率明显高于非结核患者(P值均<0.05),T-SPOT.TB、血清结核抗体、血沉检出阳性率在涂阴、涂阳结核病患者差异无统计学意义(P值均>0.05)。结论:对结核病检测敏感性:T-SPOT.TB>血沉>血清结核抗体>痰涂片,检测特异性:痰涂片>T-SPOT.TB>血清结核抗体>血沉,T-SPOT.TB检测联合多种实验室方法检测结核,能有效提高结核诊断的准确率。Objective:To explore the diagnostic values of sputum acid-fast bacilli detection,T-SPOT.TB,serum TB antibody and erythrocyte sedimentation rate(ESR)in n patients with tuberculosis(TB).Method:97 patients with tuberculosis and 27 healthy controls were recruited to perform assays of sputum acid-fast bacilli detection,T-SPOT.TB,serum TB antibody and ESR,respectively.Results:The positive rates of sputum acid-fast bacilli detection,T-SPOT.TB and serum TB antibody were significantly higher in TB patients than that in healthy individuals(all P<0.05),while no similar difference was found in ESR levels between two groups.In addition,all above four laboratory indicators were shown no differences between TB patients with positive and negative sputum acid-fast bacilli detection(all P>0.05).Conclusion:Ranking results of sensitivity for TB detection showed that,T-SPOT.TB>ESR>serum TB antibody.Ranking results of detection specificity indicated that,sputum acid-fast bacilli detection>T-SPOT.TB>serum TB antibody>ESR.T-SPOT.TB assay,in combination with the other laboratory indicators,could enhance the Diagnostic accuracy of tuberculosis.
