髋关节置换患者术后镇痛的右美托咪定应用浓度研究

摘　　要：目的讨论右美托咪定的应用浓度对髋关节置换患者术后镇痛的的影响。方法选择2016年3月~2017年12月在我院接受髋关节置换的患者70例,按随机数字表法将其分为观察组和对照组,其中观察组35例,对照组35例。两组均泵注右美托咪定,分为对照组0.3μg/(h·kg)和观察组0.6μg/(h·kg)。观察并记录两组患者术后2h(T1)、4h(T2)、8h(T3)、24h(T4)、48h(T5)五个时间点的静息状态疼痛(RVAS)、运动时疼痛(PVAS)、术后48h内胃肠道、呼吸系统、循环系统、中枢神经系统等不良反应的发生情况。结果在静息状态疼痛(RVAS)、运动时疼痛(PVAS)两者患者均有疗效,差异有统计学意义(P<0.05),但就评分而言观察组明显优于对照组。两组术后不良反应发生率相比,观察组远低于对照组,差异有统计学意义(P<0.05)。结论髋关节置换患者通过注射0.6μg/(h·kg)浓度的右美托咪定,镇痛效果良好,不良反应较小,安全性较高。Objective To discuss the effect of dexmedetomidine concentration on postoperative analgesia in patients undergoing hip replacement.Methods 70 cases of hip arthroplasty in our hospital from March 2016 to December 2017 were selected and divided into the observation group and the control group according to the random digital table method,including 35 cases in the observation group and 35 cases in the control group.The two groups were injected with dexmedetomidine and divided into control group 0.3μg/(h·kg)and observation group 0.6μg/(h·kg).Two groups of patients were observed and recorded at five time points of 2h(T1),4h(T2),8h(T3),24h(T4),48h(T5),pai n in resting state,pain during exercise,and adverse reactions of gastrointestinal tract,respiratory system,circulatory system,central nervous system,and so on.Results In resting state pain(RVAS)and exercise pain(PVAS),both patients had curative effect,with statistical significance(P<0.05).But in terms of score,the observation group was significantly better than the control group.The incidence of postoperative adverse reactions in the two groups was significantly lower than that in the control group(P<0.05).Conclusion The injection of dexmedetomidine with a concentration of 0.6μg/(h·kg)in the hip replacement patients has good analgesic effect,less side effects and higher safety.
