摘　　要：目的探讨和评估腹腔镜肝切除术(LH)治疗与开放性肝切除(OH)两种手术在围手术期疗效的差异性。方法回顾性分析2016年1月~2017年12月我院已完成的54例肝切除病例,按手术方式分为LH组和OH组,LH组31例,OH组23例;并且比较术前和术后的状况。结果 LH组手术时间、术中出血量、住院时间、术后排气时间和术后肝功能指标均低于OH组,差异均有统计学意义(P<0.05)。LH组术后并发症发生率低于OH组,差异有统计学意义(P<0.05)。两组1年复发率、2年复发率和无瘤生存率差异均无统计学意义(P>0.05)。结论腹腔镜肝切除治疗效果是安全可靠的,近期治疗效果优于开放性肝切除术,可以达到创伤小、恢复快的优点。Objective To investigate and evaluate the difference between laparoscopic hepatectomy(LH)and open hepatectomy(OH)during perioperative period.Methods A retrospective analysis of 54 cases of liver resection completed in our hospital from January 2016 to December 2017 was conducted.They were divided into LH group and OH group according to the operation mode.There were 31 cases in group LH and 23 cases in OH group,and the preoperative and postoperative conditions were compared.Results The operation time,intraoperative blood loss,hospitalization time,postoperative exhaust time and postoperative liver function index of LH group were lower than those of OH group,and the difference was statistically significant(P<0.05).The incidence of postoperative complications in the LH group was lower than that in the OH group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in 1-year recurrence rate,2-year recurrence rate and tumor-free survival rate between the two groups(P>0.05).Conclusion The therapeutic effect of laparoscopic hepatectomy is safe and reliable.The recent therapeutic effect is better than that of open hepatectomy,which can achieve the advantages of less trauma and quicker recovery.
