改良后路腰椎间融合术治疗退变性腰椎不稳的临床效果观察

摘　　要：目的采用改良后路腰椎间融合术治疗退变性腰椎不稳,观察其临床效果。方法自2013年6月~2016年3月采用改良PLIF治疗退变性腰椎不稳患者26例,所有患者定期随访,评估其JOA评分。结果 26例患者随访18~30个月,JOA评分由术前(11.38±1.6)分上升到末次随访的(27.05±1.4)分,其中优19例,良5例,中2例。所有患者术后1年CT扫描可见完全的椎间骨组织生长,完全骨性融合。结论改良PLIF治疗退变性腰椎不稳症最大限度的保留了脊柱后柱结构,创伤小,植骨融合率高,临床疗效满意。Objective To treat degenerative lumbar instability by modified posterior lumbar interbody fusion and observe its clinical effect.Methods From June 2013 to March 2016,26 patients with degenerative lumbar instability were treated with modified PLIF.All patients were followed up regularly to evaluate the JOA score.Results 26 patients were followed up for 18-30 months.The JOA score increased from(11.38±1.6)to(27.05±1.4)at the last follow-up,of which 19 were excellent,5 were good and 2 were fair.All patients underwent 1-year CT scan showed complete intervertebral bone growth,complete bone fusion.Conclusion The modified PLIF treatment of degenerative lumbar instability retained the posterior column of the maximum structure,small trauma,high fusion rate,clinical efficacy satisfactory.
