腹腔镜完全腹膜外疝修补术治疗腹股沟难复性疝的临床研究

摘　　要：目的:探讨腹腔镜全腹膜外疝修补术(TEP)在治疗腹股沟难复性疝中的临床效果及安全性评估。方法:选取2013年1月-2016年4月于本院行TEP治疗的腹股沟难复性疝患者31例为观察组,同期采用开放式后入路腹膜前腹股沟疝网片修补术治疗腹股沟难复性疝患者31例为对照组。比较两组术中出血量、手术时间、术后疼痛持续时间、住院时间、手术并发症及复发状况。结果:观察组术中出血量、疼痛持续时间、住院时间均低于对照组,手术时间长于对照组,比较差异均有统计学意义(P<0.05);观察组手术并发症发生率为6.45%,低于对照组的25.80%(P<0.05);观察组平均随访(21.5±1.5)个月,对照组平均随访(22.6±1.6)个月,观察组没有复发现象,对照组复发1例,比较差异无统计学意义(P>0.05)。结论:TEP手术治疗腹股沟难复性疝是安全可行的,可减少各种并发症和术后不适的发生率,值得临床推广应用。Objective:To evaluate the clinical effect and safety of laparoscope totally extraperitoneal prosthesis(TEP)in treatment of irreducible inguinal hernia.Method:A total of 31 patients with irreducible inguinal hernia treated by TEP in our hospital from January 2013 to April 2016 were selected as observation group,and 31 patients with irreducible inguinal hernia were treated open retroperitoneal approach for anterior inguinal hernia mesh repair at the same time were selected as control group.The intraoperative blood loss,operative time,postoperative pain duration,hospital stay,postoperative complications and recurrence between two groups were compared.Result:The intraoperative blood loss,postoperative pain duration,hospital stay in observation group were all lower than those of control group,the operation time was longer than that of control group,the differences were statistically significant(P<0.05).The incidence of operative complications in observation group was 6.45%,which was lower than 25.80%in control group(P<0.05).The average follow-up period of observation group was(21.5±1.5)months,and control group was(22.6±1.6)months,no recurrence in observation group,and 1 case recurred in control group,the difference was not statistically significant(P>0.05).Conclusion:TEP operation is safe and feasible for the treatment of irreducible inguinal hernia,can reduce the incidence of various complications and postoperative discomfort,and is worthy of clinical application.
