斜卧截石位微创经皮肾镜与输尿管软镜治疗输尿管上段结石的对比研究

摘　　要：目的:探讨斜卧截石位微创经皮肾镜(MPCNL)与输尿管软镜(FURS)的取石术治疗输尿管上段结石的临床效果。方法:选取2012年1-12月本院收治的60例输尿管上段结石患者,按手术方式将患者分为斜卧截石位MPCNL组和FURS组,各30例。比较两组治疗效果和并发症发生率。结果:FURS组术中出血、离床活动时间、术后住院时间均少于斜卧截石位MPCNL组,比较差异均有统计学意义(P<0.05);斜卧截石位MPCNL组结石清除率高于FURS组,手术时间短于FURS组,但比较差异均无统计学意义(P>0.05);斜卧截石位MPCNL组总并发症发生率为13.33%,略高于FURS组的6.67%,但差异无统计学意义(P>0.05)。结论:斜卧截石位MPCNL和FURS两种术式均有结石清除率高、并发症少的特点,但FURS比斜卧截石位MPCNL出血量更少,可缩短离床活动时间及住院时间,值得临床应用。Objective:To explore the clinical effect of minimally invasive percutaneous nephrolithotomy(MPCNL)with reclining lithotomy position and flexible ureteroscope(FURS)in treatment of upper ureteral stones.Method:A total of 60 patients with upper ureteral stones treated in our hospital from January 2012 to December 2012 were selected.According to the operative method,they were divided into MPCNL with reclining lithotomy position group and FURS group,30 cases in each group.The effect of treatment and the incidence of complications in two groups were compared.Result:The blood loss,bed stay and hospital stay of FURS group were lower than MPCNL with reclining lithotomy position group,the differences were statistically significant(P<0.05).The stone clearance rate in MPCNL with reclining lithotomy position group was higher than that of FURS group,and the operation time was shorter than that of FURS group,but the differences were not statistically significant(P>0.05).The incidence of total complications in MPCNL with reclining lithotomy position group was 13.33%,slightly higher than 6.67%in FURS group,but the difference was not statistically significant(P>0.05).Conclusion:The two types of MPCNL with reclining lithotomy position and FURS have the characteristics of high stone clearance and less complications,but FURS is less bleeding than slanting lithotomy position,can shorten the time of ambulation and stay in hospital,which is worthy of clinical application.
