B型主动脉夹层急性期低氧血症危险因素分析及干预对策

摘　　要：目的:探讨B型主动脉夹层急性期患者低氧血症的危险因素。方法:回顾性分析109例急性B型主动脉夹层患者的临床资料。低氧血症定义为平静吸氧状态下,氧合指数(PaO_2/FiO2)≤200。按此定义将患者分为低氧组(n=30)和正常组(n=79)。统计患者的性别、年龄、体质量指数(BMI)、吸烟史、高血压等既往病史,并对D-二聚体、C反应蛋白(CRP)、IL-6、肠系膜上动脉血供、肾动脉血供进行统计学检验及Logistic回归分析。结果:两组间D-二聚体、C反应蛋白、IL-6、肠系膜上动脉血供、肾动脉血供有统计学差异,Logistic回顾分析显示有关联性。结论:患者的血清D-二聚体水平、C反应蛋白、IL-6、肠系膜上动脉血供、肾动脉血供情况与发生低氧血症有关,其中D-二聚体、C反应蛋白是急性B型主动脉夹层发生低氧血症的独立危险因素。Objective:To investigate the risk of type B aortic dissection in patients with acute hypoxemia.Methods:Methods the clinical data of 109 patients with acute B type aortic dissection treated in cardiovascular surgery department were retrospectively analyzed.Hypoxemia was defined as the calm state of oxygen,oxygen index(PaO 2/FiO 2)≤200.According to this definition,patients were divided into hypoxia group(n=30)and normal group(n=79).The patient's gender,age,body mass index(BMI),smoking history,hypertension and other past medical history were counted,D-two aggregates,C reactive protein(CRP),IL-6,blood supply of superior mesenteric artery and renal artery blood supply were statistically analyzed and Logistic regression analysis was performed.Results:Between the two groups D-two dimers,C reactive protein(CRP),IL-6,superior mesenteric arterial blood supply of renal artery blood supply had significant difference,Logistic review analysis showed a correlation.Conclusion:The serum D-two level of D-dimer,C reactive protein(CRP),IL-6,superior mesenteric artery blood supply of renal artery blood supply associated with hypoxemia,including two D-dimer,C reactive protein(CRP)is an independent risk factor for acute type B aortic dissection of hypoxemia.
