高能量损伤致下肢严重开放毁损伤行保肢治疗的临床研究

摘　　要：目的:探讨高能量导致下肢严重开放毁损伤行保肢和重建治疗的临床效果。方法:选择下肢毁损伤患者共52例,根据治疗方法分为对照组22例和观察组30例,对照组行常规性手术,观察组采用保肢和重建手术;随访中位时间22.7个月,比较两组的并发症发生率、再次手术率、住院时间和经济费用,生活质量和功能康复情况。结果:观察组总并发症率为13.3%,明显低于对照组(40.91%),两组比较差异有统计学意义(P<0.05);观察组再次手术率稍微高于对照组;而观察组住院时间和经济费用均优于对照组两组,比较差异有统计学意义(P<0.05);观察组生活质量评分和下肢功能评分均优于对照组,两组比较差异有统计学意义(P<0.05)。结论:综合评估下肢毁损伤程度、医院保肢能力及患者意愿,保肢和重建治疗有一定的应用安全性和有效性。Objective:To analyze the clinical effect of limb salvage and reconstruction for severe open damage of lower extremity caused by high energy.Methods:Review of continuous selection a total of 52 cases of injury the patients were divided into the control group of 22 cases and 30 cases in the observation group,the control group underwent amputation surgery,the observation group with limb salvage and reconstruction;the median follow-up interval was 22.7 months,compared with two groups of operation the success rate,complication rate,reoperation rate,hospitalization time and economic costs,quality of life and functional rehabilitation.Results:The observation group the total complication rate was 13.3%,significantly lower than the control group(40.91%),differences(P 0.05),there was statistical significance;the observation group re operation rate was slightly higher than that in control group;the observation group hospitalization time and costs were better than the control group,were statistically difference(P 0.05),there were the statistical significance of the observation group;quality of life score and lower limb function score were higher than the control group,the differences were statistically significant(P 0.05).Conclusion:Comprehensive assessment of the degree of lower extremity damage,hospital limb salvage and patient's wishes,limb salvage and reconstruction treatment has certain application safety and effectiveness,and we need further increase sample size and follow-up time to verify.
