厄贝沙坦对糖尿病合并高血压患者血清脂联素水平影响研究

摘　　要：目的:比较血管紧张素Ⅱ受体拮抗剂厄贝沙坦与钙拮抗剂氨氯地平对2型糖尿病合并高血压患者血清脂联素(APN)水平的影响。方法:选取60例2型糖尿病合并高血压Ⅰ~Ⅱ级的患者,随机分为厄贝沙坦组30例,氨氯地平组30例。厄贝沙坦于实验之日起给予厄贝沙坦150mg,1次/d;氨氯地平组于实验之日起给予氨氯地平5mg,1次/d,共观察12周。于实验开始和结束时分别测血压、空腹血清APN,血清APN用酶联免疫吸附法(ELISA)测定。结果:治疗12周后,两组患者收缩压、舒张压均较治疗前显著下降(P<0.01);治疗后厄贝沙坦组脂联素水平较治疗前明显升高[(5.2±1.9)与(3.9±1.6)μg/ml,P<0.05],而氨氯地平组治疗前后脂联素水平无明显变化。结论:除了降压作用外,厄贝沙坦还可以提高糖尿病合并高血压患者的血清APN水平。Objective:To evaluate the effect of an ARB Erbesartan and a calcium channel blocker amlodipine on serum adiponectin levels in diabetes mellitus patients with hypertension.Methods:Sixty diabetes mellitus patients with hypertensionⅠtoⅡwere randomly divided into Erbesartan group(Erbesartan 150mg once a day,30 subjects)or amlodipine group(amlodipine 5mg once a day,30 subjects).Blood pressure,fasting serum APN were measured at baseline by the end of 12 weeks.Serum APN concentration was measured using an enzyme-linked immunosorbent assay system.Results:The systolic and diastolic BP declined significantly after the 12 weeks in both of the two groups compared to that of the baseline;Serum APN level increased significantly in Erbesartan group[(5.2±1.9 vs.3.9±1.6)μg/ml,P<0.05],whereas no significant change was found in the serum APN concentrations in amlodipine group.Conclusion:In addition to the effects of antihypertensive,Erbesartan can increase the level of APN in diabetes mellitus patients with hypertension.
