围绝经期妇女咽喉反流性疾病的治疗探索

摘　　要：目的观察激素补充疗法(hormone replacement therapy,HRT)联合质子泵抑制剂治疗围绝经期妇女咽喉反流性疾病的临床疗效,探索围绝经期妇女咽喉反流性疾病的综合治疗方案。方法将2015年1月至2018年2月因反流性咽炎就诊我院的围绝经期妇女随机分为研究组和对照组,每组45例。对照组予质子泵抑制剂口服治疗,研究组在妇科医师指导下增加激素治疗。使用RSI及RFS评分、血清胃蛋白酶原结果比较两组患者的临床疗效。结果两组患者治疗前RSI及RFS评分、血清胃蛋白酶原水平比较,差异无统计学意义(P>0.05);两组患者治疗后RFS评分及血清蛋白酶原水平比较,差异无统计学意义(P>0.05),RSI评分差异有统计学意义(P<0.05);两组患者RSI、RFS评分及PGⅠ、PGⅡ水平治疗前后比较,差异均有统计学意义(P<0.05);两组对象不良反应发生率比较,差异无统计学意义(P>0.05)。结论激素补充疗法联合质子泵抑制剂可改善围绝经期妇女咽喉反流性疾病的反流症状,有较高的应用价值。Objective To investigate the efficacy and safety of the combined use of hormone replacement therapy(HRT)of proton pump inhibitor in the treatment of perimenopausal patients with laryngopharyngeal reflex disease.Methods A total of 90 perimenopausal patients with laryngopharyngeal reflex disease presented in our hospital during January 2015 to February 2018 were randomly divided into the study group(45 cases)and the control group(45 cases).The patients in control group were treated with PPI alone and those in study group were treated PPI combined with HRT under guidance of gynaecologist.RSI,RFS scores and serum pepsinogens levels were used to evaluate efficacy after treatment.Results Before the treatment,there was no significant differences of RSI scores,RFS scores and serum pepsinogens level between the two groups(P>0.05).The RSI scores of study group was significantly lower than the scores of control group after treatment(P<0.05)and the RFS scores and serum pepsinogen level show no significant differences in the two groups(P>0.05);The RSI scores,RFS scores and serum pepsinogens level in two groups show significantly lower after the treatment.There was no significant differences in the incidence of adverse reaction(P>0.05).Conclusions HRT combined with PPI can improve the laryngopharyngeal reflex syndrome for the menopausal patients and makes high utilization in clinic treatment.
