光动力疗法治疗男性不同部位顽固性尖锐湿疣临床疗效观察

摘　　要：目的评价光动力疗法治疗男性不同部位顽固性尖锐湿疣的临床效果,探寻男性顽固性尖锐湿疣的有效治疗方法。方法收集2016年5月~2017年4月在安徽医科大学第二附属医院皮肤性病科门诊就诊的男性顽固性尖锐湿疣患者36例,其中尖锐湿疣位于阴囊2例、阴茎19例、尿道口3例、肛周12例。通过使用光动力疗法对其进行治疗,并定期随访6个月和评价临床效果。结果最终35例男性顽固性尖锐湿疣患者完成治疗和随访,尖锐湿疣平均治疗次数位于阴囊(4.50±2.12)次、阴茎(3.36±1.21)次、尿道口(4.00±1.00)次和肛周(2.18±1.66)次。阴囊部位和尿道口部位的尖锐湿疣患者均全部达到治愈,阴茎部位尖锐湿疣患者有4例复发,肛周部位尖锐湿疣患者有1例复发。比较阴茎部位和肛周部位尖锐湿疣患者的复发率,差异无统计学意义(P>0.05)。15例治愈的阴茎部位尖锐湿疣患者平均治疗次数(3.40±1.35)次,10例治愈的肛周部位尖锐湿疣患者平均治疗次数(2.30±1.70)次,两组对比差异无统计学意义(P>0.05)。治疗过程中1例患者出现轻微疼痛、局部烧灼感,6例患者出现轻度红肿现象,均于治疗后2日内消退,对治疗效果不产生影响。结论光动力疗法治疗男性不同部位顽固性尖锐湿疣具有良好的效果,肛周和阴茎部位无明显疗效差异,值得临床推广和应用。Objective To evaluate the clinical effect of photodynamic therapy on intractable condyloma acuminatum in different parts of male.Methods From May 2016 to April 2017,36 male patients with intractable condyloma acuminatum were collected from the Department of Dermatology and Venereology of the Second Affiliated Hospital of Anhui Medical University.Among them,2 cases of condyloma acuminatum were located at scrotum,19 cases at penis,3 cases at urethral orifice and 12 cases at perianal.It was treated with photodynamic therapy and was followed up for 6 months and evaluated the clinical effect.Results 35 male patients with refractory condyloma acuminatum completed the treatment and follow-up.The average number of treatments for condyloma acuminatum was scrotum(4.50依2.12)times,penis(3.36依1.21)times,urethral orifice(4.00依1.00)times and perianal(2.18依1.66)times.All the patients with condyloma acuminatum at scrotum and urethral orifice were cured,4 patients with condyloma acuminatum recurred at penis,1 patient with condyloma acuminatum around anus,the recurrence rate of condyloma acuminatum at penis and perianal area.There was no significant difference(>0.05).The average number of times of treatment for 15 cases of condyloma acuminatum at the penile area was(3.40依1.35)times.The average number of times of treatment for 10 cases of condyloma acuminatum at the perianal area was(2.30依1.70)times,and there was no significant difference between the two groups(>0.05).During treatment,1 patients had mild pain and local burning sensation,and 6 patients had mild redness and swelling.They all subsided within 2 days after treatment,and had no effect on the therapeutic effect.Conclusion Photodynamic therapy is effective in treating refractory condyloma acuminatum in different parts of the male,and there is no significant difference at perianal and penile location.It is worthy of clinical promotion and application.
