2型糖尿病患者红细胞分布宽度与外周动脉疾病相关性研究

摘　　要：目的探讨糖尿病患者红细胞分布宽度(RDW)与外周动脉疾病(PAD)之间的相关性。方法选择2015年1月~2017年1月本院综合科住院患者302例,其中糖尿病(DM)组206例,DM组按RDW四分位数分为四组。测定踝臂指数(ABI)、红细胞分布宽度(RDW)等。结果 DM组RDW水平显著高于NDM组(P<0.01)。DM组患者中。从A组到D组,ABI逐渐降低分别为,(1.03±0.15)、(0.98±0.17)、(0.97±0.18)、(0.89±0.20)。D组较A组、B组和C组差异有统计学意义(P<0.05,P<0.01);PAD患病率上升,差异有统计学意义(P<0.05);相关分析显示,RDW与ABI呈负相关(r=-0.312,P<0.01),与PAD呈正相关(r=0.257,P<0.01);Logistic回归分析结果显示:RDW为PAD的危险因素[OR=1.899,95%CI(1.299~2.778),P<0.01]。结论 DM患者RDW增高,且与ABI相关。RDW增高患者PAD风险增加。Objective To investigate the correlation between the red blood cell distribution width(RDW)and peripheral arterial disease(PAD)in diabetic patients.Methods From January 2015 to January 2017,302 cases of inpatients in our hospital were selected.There were 206 patients with diabetes(DM).The DM group was divided into four groups according to the quartiles of the RDW.Determined ankle brachial index(ABI),distribution of red blood cells(RDW)and so on.Results The RDW level in DM group was significantly higher than that in NDM group(P<0.01).For patients in the DM group,from group A to D,ABI gradually decreased(1.03±0.15),(0.98±0.17),(0.97±0.18),(0.89±0.20).There was a significant difference between group D and group A,group B and group C(P<0.05,P<0.01);the prevalence of PAD increased,the difference was statistically significant(P<0.05);correlation analysis showed that RDW and ABI Negative correlation(r=-0.312,P<0.01)was positively correlated with PAD(r=0.257,P<0.01).Logistic regression analysis showed that RDW was a risk factor for PAD[OR=1.899,95%CI(1.299~2.778),P<0.01].Conclusion The RDW in patients with DM is increased and related to ABI.Risk of PAD increased in patients with increased RDW.
