摘　　要：目的研究Castleman病(CD)的临床特点、多层螺旋CT(MSCT)表现及病理基础,以期能提高对此类病变的诊断认识。方法回顾性分析经手术病理证实的23例患者的临床、病理及MSCT影像学资料。结果多中心型CD(MCD)患者的中位年龄大于局灶型CD(LCD)患者。21例LCD患者中,病变位于胸部9例,颈部6例,腹部4例,盆腔1例,腹股沟区1例;病理诊断透明血管型19例,浆细胞型2例;MSCT表现为单发的圆形及卵圆形肿块,内可见钙化,增强后强化程度与邻近大血管相近,有些病灶边缘或病灶内见强化血管影,部分病灶内见星芒状低密度无强化区,少数病变周围有条索及絮状影。2例MCD患者,病变多发且位于全身多个部位,均被诊断为浆细胞型;MSCT示病灶强化形式低于临近大血管。结论 CD的MSCT表现与临床类型、病理分型密切相关,LCD的MSCT表现具有一定特征性,而MCD的特征性相对较少。Objective To study the clinical features,multi-slice spiral CT(MSCT)findings and pathological basis of Castleman disease(CD),in order to improve the diagnosis of such lesions.Methods The clinical,pathological and MSCT imaging data of 23 patients confirmed by operation and pathology were analyzed retrospectively.Results The median age of multicenter CD(MCD)patients was greater than that of local CD(LCD)patients.Of the 21 LCD patients,9 patients had lesions in the chest,6 in the neck,4 in the abdomen,1 in the pelvic cavity and 1 in the inguinal region;Pathological diagnosis of transparent vascular type in 19 cases and plasma cell type in 2 cases;MSCT showed solitary round and oval masses with calcification,and the enhancement degree was similar to that of adjacent large vessels after enhancement.Some lesions had enhanced vascular shadow at the edge or within the lesions,some lesions had starlike low-density and no enhancement areas,and a few lesions had stripes and flocculent shadows around the lesions.In 2 patients with MCD,the lesions were multiple and located in multiple parts of the body,all of them were diagnosed as plasmacyte type;MSCT showed that the enhancement form of lesion was lower than that of adjacent large blood vessels.Conclusion The MSCT findings of CD are closely related to the clinical types and pathological types.The MSCT findings of CD are characteristic to some extent,but the characteristics of MCD are relatively less.
