老年病区铜绿假单胞菌感染临床分析

摘　　要：目的分析老年住院患者铜绿假单胞菌感染特点及其耐药性,为临床合理使用抗菌药物提供依据。方法回顾性分析2015-2017年秦皇岛市第一医院老年病区送检标本铜绿假单胞菌(PA)分离情况、耐药性以及患者临床特点。结果2015-2017年全院共分离PA 1 479株,老年病区共分离PA 288株,占分离菌总数的34.8%,主要分离自呼吸道标本。老年病房PA分离株对12种抗菌药物耐药率皆显著高于同期非老年病房分离株(P<0.001),而敏感率显著低于非老年病房(P<0.001),老年病区分离PA的多重耐药率显著高于非老年病区(P<0.001)。老年病区多重耐药PA感染患者都合并有1-7种基础疾病且所有患者在标本送检前皆接受过抗菌药物治疗。结论铜绿假单胞菌是老年患者感染的重要病原菌,且耐药程度高,应引起实验室和临床医师高度重视。Objective To analyze the characteristics and drug resistance of Pseudomonas aeruginosa infection in elderly inpatients and provide an evidence for rational use of antimicrobial agents.Methods The isolation,drug resistance and clinical characteristics of Pseudomonas aeruginosa(PA)in geriatric ward of hospital from 2015 to 2017 were analyzed retrospectively.Results A total of 1 479 strains of PA were isolated from the whole hospital in 2015-2017,and 288 strains(34.8%)PA from the geriatric ward were isolated,mainly from respiratory tract specimens.The resistant rate of PA isolates to 12 antimicrobial agents in geriatric wards was significantly higher than that of non-geriatric ward isolates(P<0.001),and the sensitive rate was significantly lower than that of non-geriatric ward isolates(P<0.001).The rate of multidrug resistance of PA in geriatric wards was significantly higher than that in non-geriatric area(P<0.001).Patients with multidrug-resistant PA infection in the geriatric ward had 1-7 underlying diseases,and all patients received antimicrobial therapy before they were examined.Conclusion Pseudomonas aeruginosa is an important pathogen of infection in elderly patients and has higher drug resistance,which should be attached great importance to the laboratory and clinicians.
