凉血活血汤联合青黄膏对血热证寻常型银屑病患者外周血Th17细胞相关因子表达影响的分析

摘　　要：目的:探讨中药联合治疗血热证寻常型银屑病的疗效及对患者外周血中Th17细胞相关因子IL-17、IL-22、IL-23表达的影响。方法:将40例血热证寻常型银屑病患者分为凉血活血汤联合青黄膏治疗组和阿维A联合青黄膏对照组,记录治疗前后严重程度指数(PASI)评分,评价临床疗效。ELISA法检测两组患者治疗前后IL-17、IL-22、IL-23的表达水平,并与正常组(20例)进行比较,Pearson相关分析治疗前各因子水平与PASI评分的相关性。结果:治疗组与对照组治疗指数相当,差异无统计学意义(P>0.05)。治疗组和对照组治疗前IL-17、IL-22、IL-23水平均高于正常人,且与PASI评分呈正相关(P<0.01)。两组间Th17细胞相关因子表达治疗前及治疗后均无明显差异,两组治疗后上述各指标均低于治疗前(P<0.01)。结论:凉血活血汤联合青黄膏治疗血热证寻常型银屑病有较好的疗效,可能与降低患者血清中IL-17、IL-22、IL-23表达水平有关。bjective:To evaluate the efficacy of Liangxue Huoxue Decoction combined with Qinghuang Ointment for psoriasis vulgaris with blood-heat syndrome and its influence on the expression of Th17 cell-associated factors(IL-17,IL-22 and IL-23)in peripheral blood.Methods:Forty psoriatic patients with blood-heat syndrome were divided into Liangxue Huoxue Decoction combined with Qinghuang Ointment(treatment group)and acitretin A combined with Qinghuang Ointment(control group).The psoriasis area and severity index(PASI)before and after treatment were recorded,and the therapeutic efficacy was evaluated.ELISA assay was adopted to measure the expression levels of IL-17,IL-22,and IL-23 before and after the treatment in both groups,and their associations with PASI scores were analyzed.Results:The PASI scores were comparable in these two groups(P>0.05).Prior to the treatment,IL-17,IL-22,and IL-23 levels in the treatment group were higher than that in the controls,and their levels correlated positively with the PASI scores(P<0.01).Treatments lowered levels of these cytokines in both groups(P<0.01 versus before treatment in both groups).But these cytokine levels have no differences between the two groups either before or after treatments.Conclusion:Liangxue Huoxue Decoction combined with Qinghuang Ointment is effective for psoriasis with blood-heat syndrome,possibly through reduction in serum levels of IL-17,IL-22 and IL-23.
