通心络胶囊治疗冠心病的疗效及机制研究

摘　　要：目的观察通心络胶囊治疗冠状动脉粥样硬化性心脏病(冠心病)心肌缺血的临床效果。方法选取商丘市第一人民医院心内科于2015年2月~2017年1月收治的冠心病不稳定性心绞痛患者108例,随机分成对照组(54例)和实验组(54例),对照组患者采用西药常规治疗;实验组患者在对照组基础上,给予通心络胶囊治疗,比较两组患者临床疗效、血脂水平改变情况、血管内皮功能及炎症因子的变化情况。结果治疗后,两组患者在胸痛(心绞痛)、胸闷、心悸、气短、乏力临床症状方面,实验组均显著优于对照组(P<0.05);实验组内皮素-1(ET-1)、血栓素(TXB2)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)水平下降,一氧化氮(NO)、低密度脂蛋白胆固醇(HDL-C)水平上升程度,实验组均明显优于对照组,差异有统计学意义(P<0.05);两组肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)和C反应蛋白(CRP)水平明显下降,实验组下降程度优于对照组,差异具有统计学意义(P<0.05)。结论通心络胶囊治疗冠心病心肌缺血,可显著改善患者临床症状,降低炎性反应,改善患者血管内皮细胞损伤程度。Objective To observe the clinical efficacy of Tongxinluo Capsule in treatment of myocardial ischemia of coronary heart disease(CHD).Methods The patients with unstable angina pectoris(UAP)of CHD(n=108)were chosen from the First Department of Cardiology of the First People’s Hospital of Shangqiu City from Feb.2015 to Jan.2017,and then randomly divided into control group and test group(each n=54).The control group was given routine treatment of Western medicine,and test group was additionally given Tongxinluo Capsule.The clinical curative effect,and changes of blood fat level,vascular endothelial function and inflammatory factors were compared between 2 groups.Results After treatment,the reliefs of clinical symptoms including chest pain(angina pectoris),chest distress,palpitation,shortness of breath,hypodynamia were more significant in test group than those in control group(P<0.05).The levels of endothelin-1(ET-1),thromboxane B2(TXB2),total cholesterol(TC),triglyceride(TG)and low-density lipoprotein-cholesterol(LDL-C)decreased,and levels of nitric oxide(NO)and high-density lipoprotein-cholesterol(HDL-C)increased in test group compared with control group(P<0.05).The levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and C-reactive protein(CRP)decreased significantly in 2 groups,which was more significant in test group compared with control group(P<0.05).Conclusion Tongxinluo Capsule can significantly relieve clinical symptoms,reduce inflammatory reactions and ameliorate severity of vascular endothelial cell injury in treatment of myocardial ischemia of CHD.
