苏源冰运用手法整复配合中药治疗旋后外旋型踝关节骨折的临床疗效观察

摘　　要：目的:观察苏源冰运用手法整复配合中药治疗旋后外旋型踝关节骨折的临床疗效。方法:回顾分析2016年1月-2017年3月,苏源冰副主任医师运用手法整复旋后外旋型踝关节骨折45例,并配合中药治疗,积极功能锻炼,观察骨折复位情况和后期临床疗效。结果:根据踝关节骨折复位放射学评价标准(Burwell-Charnley):32例Ⅱ度骨折患者,有21例达解剖复位,另外复位可11例;5例Ⅲ度骨折患者中,达解剖复位者1例,复位可3例,另外有1例复位差;8例Ⅳ度骨折患者中,达解剖复位者3例,复位可3例,复位差2例。按美国足踝协会(American Orthopaedic Foot&Ankle Society,AOFAS)推荐的踝与后足功能评分标准评定患足功能,具体标准为:优为不低于90分,良为75~89分,可为50~74分,差为低于50分。本组优33足,良7足,可3足,差2足,优良率为88.9%。结论:手法整复配合中药治疗旋后外旋型踝关节骨折,简单易学,运用得当疗效满意,仍有临床应用价值。Objective:To observe the efficacy of manual reduction plus TCM medicine on supination-eversion fractures of ankle.Methods:45 cases were treated by manual reduction plus TCM medicine and active functional exercise;the fracture reduction and late clinical efficacy were observated.Results:According to the Burwell-Charnley criteria,in 32 patients with grade II-degree fractures,21 cases were anatomic reduction and reduction of 11 cases.In 5 cases of III-degree fractures,1 case was anatomic reduction and reduction of 11 cases,and 1 case of poor reset.Among the 8 cases of IV-degree fracture,3 cases were anatomic reduction,reduction of 3 cases,and 2 cases of poor reset.According to the AOFAS recommended squat and hind foot function scoring,33 feet were excellent,7 feet of good,3 feet of little better and 2 feet of worse.The excellent and good rate was 88.9%.Conclusion:Manual reduction plus TCM medicine on supination-eversion fractures of ankle shows a satisfactory result,easy to learn,and is still clinical application value.
