摘 要:高效、合理、规范的住院医师规范化培训(住培)管理是提高培训质量并实现均质化目标的重要保障。深圳市儿童医院将PDCA循环管理方法运用于儿科住培管理,针对住培实际管理工作中遇到的问题,结合师生座谈会及问卷调查反馈情况,采用"头脑风暴""排列图(柏拉图)""鱼骨图(因果图)"等常用管理工具,分析现状、发现问题并查找原因。当前儿科住培管理存在的主要问题如下:轮转管理不规范、住院医师对薪酬待遇不满意、师资培训不到位、师资评价及激励机制不健全等。住培办公室针对上述问题拟定相应的对策、制定详尽的执行方案、严格执行工作计划,并指定专人负责并对落实情况定期督导跟进。经过2轮PDCA循环,在规范轮转管理、增强师资队伍建设、实现同工同酬、健全师资评价及激励机制等方面取得长足进步。培训基地对成功的经验加以肯定,并予以标准化(更新制度流程),并将遗留问题带入下一PDCA循环,住培管理工作得到科学化的持续发展。Effective,reasonable and standardized management of standardized residency training(SRT)program is an important guarantee to improve training quality and achieve homogenization.Shenzhen children's hospital applied PDCA cycle management method in the management of SRT.According to the problems encountered in the practice of management of SRT,we combined the feedbacks from the exchange meetings and questionnaires from teachers and students.We also analyzed the current situations,found problems and discovered the causes by using common management tools including brainstorming,arrangement drawing(Plato),fishbone drawing(causality drawing).The key problems are identified as followed.Firstly,the rotation management is not standardized,and the residents are not satisfied with the income.Secondly,the training for teachers is insufficient,and the evaluation and incentive system for teachers are not perfect.The SRT office developed corresponding strategies,strictly implemented the work plan,and appoint specially-assigned person to be in charge of the supervision of implementation regularly.After 2 cycles of PDCA,great progress has been made in the management of pediatric SRT,strengthening the construction of training faculty,and achieving equal pay for equal work.There was also a great progress in improving evaluation and incentive system for teachers.The training base had a positive appraisal for our successful experience,and tried to standardize it(updating the regulations and processes).The legacy issues were also brought into the next PDCA cycle.The management of pediatrics’SRT has been scientifically and steadily developed.
