摘　　要：目的探讨核苷酸结合寡聚化结构域样受体 3 (nucleotide-binding oligmerization domain like receptor family pyrin domain-containing 3,NLRP3)炎性体表达水平在冠心病患者心室重塑中的作用。方法选取2016年5月至2017年2月本院收治的冠心病患者108例纳入观察组,另选取同期本院健康体检者108例纳入对照组。比较两组研究对象血清中NLRP3、凋亡相关微粒蛋白(apoptosis-associated speck-like protein containing CARD,ASC)、半胱氨酸蛋白酶-1(caspase-1)、白介素(IL)-1β、IL-18表达水平及平均室壁应力、左室心肌质量指数、左室收缩末容积、左室射血分数、左室舒张末容积,分析NLRP3炎性体、ASC、caspase-1、IL-1β、IL-18表达水平与平均室壁应力、左室心肌质量指数、左室收缩末容积、左室射血分数、左室舒张末容积的相关性。结果观察组患者血清中NLRP3炎性体、ASC、caspase-1蛋白质相对表达量、IL-1β和IL-18表达水平、平均室壁应力、左室心肌质量指数、左室收缩末容积、左室舒张末容积均显著高于对照组(P<0.05),左室射血分数显著低于对照组(P<0.05)。Pearson线性相关性分析显示,NLRP3炎性体、ASC、caspase-1、IL-1β、IL-18表达水平与平均室壁应力、左室心肌质量指数、左室收缩末容积、左室舒张末容积均呈正相关(P<0.05)。结论冠心病患者NLRP3炎性体通路被激活,与心室重塑有关。Objective To investigate the role of nucleotide-binding oligmerization domain like receptor family pyrin domaincontaining 3(NLRP3)inflammasome expression in ventricular remodeling in patients with coronary heart disease(CHD).Method From May 2016 to February 2017,108 cases of patients with coronary heart disease in our hospital were prospectively collected as observation group,and 50 cases of healthy subjects in our hospital were collected as control group during the same time.The serum levels of NLRP3 inflammasome,apoptosis-associated speck-like protein containing CARD(ASC),cysteine aspartic proteinase 1(caspase-1),IL-1β,IL-18,mean ventricular wall stress,left ventricular mass index,left ventricular end systolic volume,left ventricular ejection fraction,and left ventricular end diastolic volume were compared between the two groups,and the relationships of NLRP3 inflammasome,ASC,caspase-1,IL-1β,IL-18 and mean ventricular wall stress,left ventricular mass index,left ventricular end systolic volume,left ventricular ejection fraction,left ventricular end diastolic volume were studied.Result The relative expression of NLRP3 inflammasome,ASC and caspase-1 protein,IL-1βand IL-18,the mean ventricular wall stress,left ventricular mass index,left ventricular end systolic volume and end diastolic volume in observation group were higher than control group(P<0.05),the left ventricular ejection fraction was lower than control group(P<0.05).Pearson linear correlation analysis showed that NLRP3 inflammasome,ASC,caspase-1,IL-1βand IL-18 had significant positive correlations with mean ventricular wall stress,left ventricular mass index,left ventricular end systolic volume,and left ventricular end diastolic volume(P<0.05).Conclusion NLRP3 inflammasome pathway is activated in patients with coronary heart disease and is related to ventricular remodeling.
