摘 要:目的:观察脉冲振荡测定在胸闷变异型哮喘患者中的应用效果。方法:随机选取确诊的30例胸闷变异型哮喘(CTVA)患者为CTVA组,同时纳入同期健康体检者28例为对照组,两组患者同时行肺通气功能法和IOS法两种检查,比较两组检测结果。结果:基础状态下,肺通气法测定时,除FVC%pred外,CTVA组FEV1%pred、FEV1/FVC%、PEF%pred、FEF25%pred、FEF50%pred、FEF75%pred、MMEF%pred等其他肺通气功能参数均明显低于对照组,差异均有统计学意义(P<0.05);IOS法测定时,除R 20%外,CTVA组Zrc%、Fres、R 5%、R 5-R 20、X5等参数均明显高于对照组,差异均有统计学意义(P<0.05);CTVA组IOS的各参数与肺通气功能参数有显著相关性,其中Zrc%、R5%与PEF%pred均有负相关性。结论:脉冲振荡测定在胸闷变异型哮喘患者中的应用有助于提高诊断准确率,降低误诊率和漏诊率。Objective:To observe effects of impulse oscillometry(IOS)in patients with chest tightness variant asthma(CTVA).Methods:The randomly selected and conformed 30 CTVA patients were set as CTVA group.At the same time,28 cases for health examination were used as control group.The two groups were given pulmonary ventilation function test and IOS,and the results were compared.Results:In the basic state,Conclusion of the pulmonary ventilation function test indicated that in addition to FVC%pred,the levels of the other pulmonary ventilation function parameters including FEV1%pred,FEV1/FVC%,PEF%pred,FEF25%pred,FEF50%pred,FEF75%pred,and MMEF%pred in the CTVA group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).Determination of IOS shows:except for R 20%,the levels of ZVA%,Fres,R 5%,R 5-R 20,X5 and other parameters in the CTVA group were significant higher than those in the control group,and the differences were statistically significant(P<0.05).There were significant correlations among the parameters of the IOS and the lung ventilation function parameters in the CTVA group,wherein,Zrc%and R5%were negatively correlated with PEF%pred.Conclusions:The application of impulse oscillometry in the patients with CTVA can help improve the diagnostic accuracy and reduce the misdiagnosis rate and missed diagnosis rate.
