摘　　要：目的探讨围手术期集束化呼吸道护理对颈椎骨折手术患者肺部并发症的影响。方法选择福建医科大学附属第二医院骨科2010年1月—2015年12月经颈椎前路手术的颈椎骨折患者为研究对象,以开展围手术期集束化呼吸道护理时间为界分为集束化组及对照组,对照组实施常规呼吸道护理,集束化组实施集束化呼吸道护理,比较两组患者围手术期肺部并发症的发生率。结果集束化组43例患者经围手术期集束化呼吸道护理,有5例出现肺部并发症,其中2例出现痰栓性肺不张,3例出现肺部感染(1例合并呼吸功能衰竭);对照组22例患者,7例肺部并发症,其中3例出现痰栓性肺不张,4例出现肺部感染(2例合并呼吸功能衰竭),集束化组与对照组的肺部并发症发生率比较差异有统计学意义(P<0.05)。结论实施围手术期集束化呼吸道护理有助于降低颈椎骨折经颈椎前路手术患者肺部并发症的发生率。Objective To investigate the effect of perioperative bundled respiratory tract care on pulmonary complications in patients undergoing cervical fracture surgery.Methods Patients with cervical spine fractures undergoing anterior cervical surgery from January 2010 to December 2015 were selected as the subjects of the Department of Orthopaedics,Second Affiliated Hospital of Fujian Medical University.They were divided into a bundled group and a control group according to the time of the perioperative bundled respiratory care.The control group was treated with routine respiratory care,and the bundled group was treated with a bundled respiratory care.The incidence of perioperative pulmonary complications was compared between the two groups.Results In the bundled group,43 patients received perioperative bundled respiratory care,and 5 patients had pulmonary complications.Among them,2 cases had sputum atelectasis and 3 cases had pulmonary infection(1 case with respiratory failure).In the control group,22 patients and 7 pulmonary complications,3 cases of sputum atelectasis,4 cases of pulmonary infection(2 cases with respiratory failure).The rate of pulmonary complications in the bundled group and control group was significantly different(P<0.05).Conclusion Implementing perioperative bundled respiratory care is contribute to reduce the incidence of pulmonary complications in patients undergoing anterior cervical surgery for cervical spine fractures.
