SBAR标准化交接单在产科一级护理患者交接班中的应用

摘　　要：目的探讨SBAR标准化交接单在产科一级护理患者中的应用效果。方法将2017年6—7月产科一级护理患者48例进行的240次交班设为对照组,将2017年8—9月产科一级护理患者62例进行的310次交班设为试验组。对照组采用传统的交接班模式,晨会医护大交班,床头交接班按医院交接班制度规定需要交接的内容进行交接;试验组在晨会上进行以SBAR报告形式对本组患者进行晨会交接班,在床边以标准化SBAR床边交接单进行逐项床边交接,完成交接后逐项打勾并签字。比较两组存在问题发生情况及对病情的知晓情况。结果试验组存在问题情况明显少于对照组(P<0.05),护士对试验组患者的病情知晓情况高于对照组(P<0.05)。结论使用标准化SBAR床边交接单有利于规范交接班流程,使交接班过程有着统一标准、规范流程,逐一对照内容防止交接时遗漏,并且可以在交接时及时发现上一班次存在问题和缺陷点,标准化SBAR床边交接单使用在晨会报告病情时,提高了护士的沟通能力、表达能力及对病情的知晓率,长此以往提高了护士的专业水平,可以及早发现病情变化,医师和患者对护士工作的满意度提高。Objective To explore the application effect of SBAR standardized handover sheet used by obstetrical patients with the first grade nursing.Methods To compared the occurrence of existing problems and knowledge of the state of patients,took the 240 shifts of the 48 cases of patients with obstetrical first grade nursing from June to July 2017 as the control group,310 shifts of 62 cases of patients with obstetrical first grade nursing from August to September 2017 as the study group.The control group adopted the traditional shift mode,the so-called morning care shift,the bedside shift be handed over according to the requirements of the hospital shift system.The study group adopted the SBAR report mode in the morning care shift,and bedside shift was also carried out with the standardized SBAR handover sheet to confirm every shift.After the shift was completed,each item be ticked for insurance with signature.Results There were fewer problems in the study group than in the control group(P<0.05),and nurses'knowledge about the patients'state was higher than the control group(P<0.05).Conclusion The adoption of standardized SBAR bedside handover sheet proved to be of great help to standardize and unify the shifting process.The method of contrasting the contents one by one was helpful to prevent omission during the handover,and could also discover the problems and defects in the last shift in time.The application of standardized SBAR bedside handover sheet in the morning care report had greatly improved nurses'communication ability,expression ability,and the awareness of the patients'state.In the long run,hereby nurses'professional ability were promoted,the changes of patients'state could be detected as soon as possible,and could also improve doctors and patients'satisfaction for the nurses'work.
