米非司酮联合曼月乐在围绝经期异常子宫出血的效果观察

摘　　要：目的米非司酮联合曼月乐在围绝经期女性异常子宫出血的临床效果观察。方法收集56例围绝经期异常子宫出血合并有轻中度贫血的患者,随机分为对照组及研究组,各28例。所有患者均为宫腔镜术后第5天,口服米非司酮12.5 mg,1次/d,3个月为1个疗程。研究组添加曼月乐治疗。治疗后随访6~12个月,对比两组的治疗过程中,子宫内膜厚度及血红蛋白、不良反应等情况。结果两组治疗前后及治疗后6个月血红蛋白、子宫内膜情况,差异有统计学意义(P<0.05),两组治疗后与治疗前差异均有统计学意义(P<0.05),随访治疗后6个月研究组优于对照组,差异有统计学意义(P<0.05),均无严重不良反应,差异无统计学意义。结论米非司酮联合曼月乐治疗围绝经期异常子宫出血效果较好,优于单用常规剂量米非司酮。Objective To study the effects of mifepristone combined with Mirena on perimenopausal dysfunctional innormal uterine bleedind.Methods 56 permenopausal women with dysfunctional innormal uterine bleeding with low hemoglobin in department of our hospital were selected and divided into two groups according to the random number table method.28 cases in control group received the treatment of mifepristone,and 28 cases in treatmental group received the treatment of mifepristone combined with Mirena.Five days after hysteroscope opreation,all cases were treated with mifepristone(12.5 mg daily)for three months.The treatmental group also received the treatment of Mirena.After 6-12 months,follow-up,the clinical efficacy in two groups was compared on the endometrial thickness,blood hemoglobin level and unrealized reactions.Results In the two groups,the endometrium became thinner after treatment,that of study group was much thinner(P<0.05).The blood hemoglobin level of both groups increased after treatment,that of study group was much better(P<0.05).During the treatment period,few cases occur adverse reactions,there was no statistically significant between two groups.Conclusion For patients with perimenopausal dysfunctional innormal uterine bleedind,the treatment method of mifepristone combined with Mirena can improve the symptom of bleeding,increase hemoglobin concentration,reduce the thickness of the endometrium and adverse reactions,so it is worthy of popularization and application.
