摘　　要：目的探讨心钠素(ANP)、脑利钠肽(BNP)、C型利钠肽(CNP)及血小板内皮细胞黏附分子1(PECAM-1)在老年2型糖尿病(type 2 diabetes mellitus,T2DM)血管病变患者血浆中的水平及临床意义。方法选取2011年1月—2016年12月石家庄市第一医院收治的T2DM 116例,根据是否合并血管病变将其中未合并血管病变的73例作为单纯T2DM组,合并血管病变的43例作为T2DM血管病变组,另选取该院同期老年健康体检者52例作为对照组,观察比较3组血浆ANP、BNP、CNP、PECAM-1水平,分析T2DM血管病变组血浆ANP、BNP、CNP水平与PECAM-1水平的相关性。结果 3组血浆ANP、BNP、CNP及PECAM-1水平总体比较差异有统计学意义(P<0.05)。T2DM血管病变组血浆ANP、BNP和PECAM-1水平显著高于对照组和单纯T2DM组,血浆CNP水平显著低于对照组和单纯T2DM组,差异均有统计学意义(P<0.05)。单纯T2DM组血浆CNP和PECAM-1水平高于对照组,差异有统计学意义(P<0.05)。T2DM血管病变组血浆ANP、BNP水平与PECAM-1水平呈显著正相关,血浆CNP水平与PECAM-1水平呈显著负相关。结论血浆ANP、BNP、CNP及PECAM-1水平在老年T2DM血管病变患者中可发生显著变化,可作为诊断老年T2DM患者血管病变的可靠指标。Objective This study was designed to explore the levels and clinical significance of atrial natriuretic peptide(ANP),brain natriuretic peptide(BNP),C-type natriuretic peptide(CNP),and platelet endothelial cell adhesion molecule 1(PECAM-1)in the plasma of patients with type 2 diabetes(T2DM)complicated by angiopathy.Methods We selected 116 cases of T2DM patients who received treatment in The First Hospital of Shijiazhuang City.According to presence or absence of vascular diseases,73 patients without angiopathy were treated as simple T2DM group and 43 patients with angiopathy as angiopathy group,and 52 cases of healthy subjects who were treated during the same period were selected as a control group.Levels of ANP,BNP,CNP,and PECAM 1 were observed and compared in the three groups,and the correlation between levels of ANP,BNP and CNP and the level of PECAM 1 in T2DM angiopathy group was analyzed.Results Levels of plasma ANP,BNP,CNP,PECAM-1 were statistically significant in the three groups(P<0.05).The levels of plasma ANP,BNP and PECAM-1 in T2DM angiopathy group were significantly higher than those in the control group and simple T2DM group.In addition,the plasma CNP level was significantly lower than that of the control group and the simple T2DM group(P<0.05).The levels of CNP and PECAM-1 were remarkably higher in angiopathy group than in control group(P<0.05).The levels of plasma ANP and BNP were positively correlated with PECAM-1 in T2DM angiopathy group,and the level of CNP was negatively correlated with PECAM-1(P<0.05).Conclusion The levels of ANP,BNP,CNP and PECAM-1 could be significantly changed in the elderly patients with T2DM complicated by angiopathy,which can be used as a sound indicator for the diagnosis of angiopathy in elderly patients with T2DM.
