摘 要:目的:检测反复呼吸道感染儿童血清维生素A水平,探讨儿童边缘型维生素A缺乏的危险因素。方法:选择2017年1月至10月在我院就诊的呼吸道感染儿童74例,同期体检的健康儿童68例为研究对象。比较不同临床诊断儿童血清视黄醇水平及边缘型维生素A缺乏的发生率;比较反复呼吸道感染儿童和非反复呼吸道感染儿童血清视黄醇水平及边缘型维生素A缺乏的发生率;并通过Logistic回归分析儿童边缘型维生素A缺乏的危险因素。结果:反复呼吸道感染现症感染儿童、反复呼吸道感染非现症感染儿童、急性呼吸道感染儿童的血清视黄醇水平均低于健康儿童,差异均有统计学意义(P<0.05)。反复呼吸道感染现症感染儿童边缘型维生素A缺乏的发生率高于健康儿童(P<0.05)。反复呼吸道感染儿童血清视黄醇水平低于非反复呼吸道感染儿童;边缘型维生素A缺乏的发生率则高于非反复呼吸道感染儿童(P<0.05)。经Logisitic回归分析,食欲欠佳、农村居住、反复呼吸道感染是儿童边缘型维生素A缺乏发生的独立危险因素。结论:边缘型维生素A缺乏与反复呼吸道感染有一定的相关性;且受机体免疫功能低下、居住环境等因素影响。Objective:To detect serum vitamin A levels in children with recurrent respiratory tract infection and to explore the risk factors of marginal vitamin A deficiency in children.Methods:A total of 74 children with respiratory tract infection treated in our hospital and 68 healthy children in the same period were selected as the subjects.Serum retinol levels and incidence of marginal vitamin A deficiency in children with different clinical subtypes of respiration infection were compared.The serum retinol levels and the incidence of marginal vitamin A deficiency in children with recurrent or non-recurrent respiratory tract infection were compared and the risk factors of marginal vitamin A deficiency in children were analyzed by Logistic regression analysis.Results:The serum retinol levels of children in acute infection with recurrent respiratory tract infection,non-acute infection with recurrent respiratory tract infection and acute respiratory infection were lower than those of healthy children(P<0.05).The incidence of marginal vitamin A deficiency in children with recurrent respiratory infection was higher than that of healthy children(P<0.05).The level of retinol in children with recurrent respiratory infection was lower than that of children with non-recurrent respiratory infection,and the incidence of marginal vitamin A deficiency was higher than that of non-recurrent respiratory tract infection(P<0.05).Logistic regression analysis showed that poor appetite,rural residence and recurrent respiratory tract infection were independent risk factors for marginal vitamin A deficiency in children.Conclusion:Marginal vitamin A deficiency is associated with recurrent respiratory infection,and is affected by low immune function,living environment and other factors.
