摘　　要：目的探讨湿性脂肪干细胞辅助自体脂肪颗粒注射移植在面部轮廓重塑中的应用,观察其疗效。方法选取25例先天或后天性因素导致额部、颊部、眶周、颏部、颞部等凹陷、形态不良的患者,抽取大腿部脂肪颗粒,静置、纯化等方法处理,将未分离的脂肪间充质干细胞(adipose-derived stem cells,ADSC)与基质血管成分(stromal vascular fraction,SVF)复合脂肪颗粒,30%超量多层次多隧道均匀注射于面部标记区域。每病例手术1~3次,间隔3~6个月。通过比较术前及术后照片、脂肪吸收情况和患者自我评价等指标进行效果评价。结果 25例患者共86处手术部位,随访3~24个月,效果较满意,无脂肪液化、感染破溃等并发症。结论湿性脂肪干细胞辅助的脂肪移植应用于面部轮廓重塑的治疗操作简单、效果满意、创伤小、患者容易接受,手术效果理想。Objective To investigate the application of wet fat stem cell assisted autologous fat granule injection in the treatment of facial contour reconstruction,and to observe the curative effect.Methods Collected the clinic data of 25 patients who manifested depression deformity or morphological abnormality in frontal,temporal buccal,periorbital or submental region.Harvested the autologous fat granules from patient's thighs,then rest and purifi ed the granules.Injected the unsegregated mixed fat particles of Adipose-derived stem cells and Stromal Vascular Fraction into the marked facial regions by the way of multiple levels and tunnels with an amount of more than 30%.The procedure had been done 1 to 3 times in each case with a 3 to 6 months interval period.The effi cacy was evaluated by comparing with the photo differences,fat absorption degree and patient self-evaluation before and after surgeries.Results A satisfactory and long lasting result had been observed in all the 25 patients with 86 injection spots.During the 3 to 24 months follow-up,no fat liquefaction,infection and other serious complications had been observed.Conclusion The application of wet fat stem cell assisted autologous fat granule injection in the treatment of facial contour reconstruction has advantages of easy operation,satisfactory result and minimal invasion.It is easy to be accepted by patients,also has an ideal clinical application.
