半月神经节手动脉冲射频治疗三叉神经带状疱疹后神经痛临床研究

摘　　要：目的:回顾性分析研究半月神经节长时程脉冲射频治疗三叉神经带状疱疹后神经痛的临床疗效。方法:28例病程在1~3个月之间的三叉神经分布区带状疱疹后神经痛病人,给予CT引导下长时程半月神经节手动脉冲射频治疗,记录病人术前、术后1 d、7 d、30 d、90 d、180 d加巴喷丁使用剂量、视觉模拟评分法(visual analogue scale,VAS)及匹兹堡睡眠质量指数(Pittsburgh sleep quality index,PSQI),并记录不良反应的发生情况。结果:病人术后各时间点加巴喷丁使用剂量明显减少(P<0.001),VAS评分明显降低(P<0.001),PSQI明显降低(P<0.001),整个治疗过程中未见明显不良反应发生。结论:半月神经节长时程手动脉冲射频治疗三叉神经带状疱疹后神经痛,能有效改善疼痛及睡眠状况,提高了病人生活质量,适用于临床治疗。Objective:This study aimed to observe the safety and clinical effect of long-duration manual pulsed radiofrequency on casserian ganglion for the treatment of trigeminal postherpetic neuralgia.Methods:28 patients diagnosed with the trigeminal herpes zoster for 1-3 months were collected,who received the treatment of CT-guided long-duration manual pulsed radiofrequency on casserian ganglion,and the therapeutic effects were evaluated by gabapentin dose,visual analogue scale(VAS)and the Pittsburgh sleep quality index(PSQI)before the treatment and 1 d,7 d,30 d,90 d,180 d after the treatment.Results:The use of gabapentin dose,VAS score and the PSQI score at each time point after the PRF treatment were significantly lower than before the treatment(P<0.001).No serious complications were identified during the treatment.Conclusion:The long-duration manual pulsed radiofrequency on casserian ganglion is a safe and effective therapeutic alternative for trigeminal postherpetic neuralgia patients.This treatment can effectively reduce pain and relieve the sleep disorder and improve the quality of life of patients.
