运动试验和动态心电图诊断冠心病的临床价值分析

摘　　要：目的探讨运动试验和动态心电图诊断冠心病的临床价值。方法随机选取2015年3月至2017年4月医院收治临床拟诊为冠心病的患者80例,均行运动心电图和24h动态心电图检查的方式实施诊断研究,其中以冠状动脉造影作为标准对2种检查方式下的相关临床价值进行对比。结果平板运动试验对冠心病疾病进行诊断时,敏感性为72.2%,特异性为80.1%;12导联动态心电图诊断时,诊断的敏感性为63.8%,特异性为75.3%。平板运动试验和12导联动态心电图,2种方式在冠心病诊断时,其敏感性和特异性之间差异不明显(P>0.05),对冠脉不同支数病变阳性的检出率差异不明显(P>0.05),冠脉双支及多支病变的阳性检出率要比单支病变检出率更高(P<0.05)。结论平板运动试验和动态心电图等是当前冠心病诊断相对简便实用和安全可靠的诊断方式,同时也具有无创的特点,在进行冠脉双支及多支病变情况诊断时,其阳性检出率相对比较高;在2种检查方式相结合进行诊断时,对冠心病诊断的准确性可有效提升。Objective To investigate the clinical value of exercise test and dynamic electrocardiogram in the diagnosis of coronary heart disease.Methods Randomly selected from our hospital from March 2015 to April 2017 clinical examination for a total of 80 patients with coronary heart disease(CHD)as the study object,exercise electrocardiogram in patients with these 24 h dynamic ecg examination method for diagnosis and research,among them with coronary angiography as a standard for two kinds of inspection of the relevant clinical value were analyzed.Results The exercise treadmill test to diagnose coronary disease,the sensitivity was 72.2%,the specificity of display was 80.1%,the dynamic of 12 lead ecg diagnosis,the diagnostic sensitivity of 63.8%,the specificity of 75.3%.Two ways in the diagnosis of coronary heart disease,no obvious difference between the sensitivity and specificity(P>0.05),the diagnosis of exercise treadmill test method and the dynamic of 12 lead ecg diagnosis form for coronary counts lesion detection rate,the difference was not obvious(P>0.05),the exercise treadmill test and dynamic electrocardiogram in CHD examination,positive detection rate of coronary double teams and multivessel lesions than single lesion detection rate was higher(P<0.05).Conclusion The exercise treadmill test and the dynamic electrocardiogram is currently the diagnosis of coronary heart disease diagnosis is relatively simple and practical and safe and reliable way,at the same time also has the characteristics of non-invasive,double and multiple branch in the coronary artery lesions of diagnosis,the positive detection rate is relatively high,the two kinds of inspection methods for diagnosis,effective improve diagnostic accuracy of CHD.
