地屈孕酮片治疗先兆流产的效果及对妊娠结局的影响

摘　　要：目的:探讨地屈孕酮片治疗先兆流产的临床效果及对妊娠结局的影响。方法:选择2016年1月-2018年1月100例先兆流产患者,随机分为观察组和对照组,各50例。对照组肌注黄体酮20 mg/次,1次/d,观察组口服地屈孕酮片治疗,口服首次剂量40 mg,然后每隔8小时口服10 mg。两组用药时间均至流产症状消失。比较两组疗效、临床症状消失时间(止血时间、腹痛缓解时间)及激素(孕激素、人绒毛膜促性腺激素)水平,观察两组妊娠结局(流产、早产、足月妊娠)。结果:观察组治疗有效率为96.00%,高于对照组的80.00%,差异有统计学意义(P<0.05)。观察组止血时间为(3.82±0.80)d,腹痛缓解时间为(4.08±1.23)d,均短于对照组的(4.72±0.83)d、(5.04±1.07)d,差异均有统计学意义(P<0.05)。治疗前两组孕激素、人绒毛膜促性腺激素比较,差异无统计学意义(P>0.05),治疗后观察组均高于对照组,差异均有统计学意义(P<0.05)。观察组流产例数少于对照组,足月妊娠例数多于对照组,差异均有统计学意义(P<0.05),两组早产例数比较差异无统计学意义(P>0.05)。结论:地屈孕酮片治疗先兆流产有效率高,缩短临床症状消失时间,提高P、β-h CG水平,改善妊娠结局。Objective:To investigate the clinical effect of Dydrogesterone tablets in treatment of threatened abortion and its effect on pregnancy outcome.Method:A total of 100 patients with threatened abortion,selected from January 2016 to January 2018,were randomly divided into the observation group and the control group(50 cases in each group).The control group was injected with progesterone,20 mg/times,1 time/d,the observation group was treated with dydrogesterone tablets,the first dose of 40 mg was taken,and then 10 mg was taken every other 8 h.The medication time was stopped when the abortion symptoms were disappearance in the two groups.The efficacy,clinical symptom disappearance time(hemostasis time,abdominal pain relief time),the level of hormone(progesterone,human chorionic gonadotropin)were compared in the two groups,and pregnancy outcomes(abortion,premature birth,full-term pregnancy)were observed in the two groups.Result:The effective rate was96.00%in the observation group,which was higher than 80.00%in the control group,the difference was statistically significant(P<0.05).The time of hemostasis in the observation group was(3.82±0.80)d,and the time of abdominal pain relief was(4.08±1.23)d,which were shorter than that of the control group(4.72±0.83)d and(5.04±1.07)d,the differences were statistically significant(P<0.05).There was no significant difference in progesterone and chorionic gonadotropin between the two groups before treatment(P>0.05).The observation group was higher than the control group after treatment,and the difference was statistically significant(P<0.05).The number of abortion in the observation group was less than that in the control group,and the number of term pregnancy was more than that in the control group(P<0.05),but there was no significant difference in the number of premature delivery between the two groups(P>0.05).Conclusion:Dydrogesterone tablets can improve the effective rate,shorten clinical symptom disappearance time,improve the level of P andβ-HCG,and improve pregnancy outcome
