辅助生殖中精子获能时间对精子质量参数的影响

摘　　要：目的探讨优化处理后精液孵育至授精(精子体外获能)时间对精液质量参数的影响,从而更有效地提高辅助生殖技术成功率。方法收集在本中心就诊的400例男性的精液,其中正常精子、少精子症、弱精子症和畸形精子症男性各100例,密度梯度离心法处理后马上置5%CO_2、37℃、95%湿度培养箱孵育,分别记录20、40、80和120 min时的精液参数。结果弱精子症组精液在孵育20 min后,前向运动(PR)精子百分率、PR精子总数、精子总活力(PR+NP%)和活动精子总数显著下降,差异有统计学意义(P=0.000、P=0.000、P=0.000和P=0.000),正常精子组、少精子症组和畸形精子症组精液在孵育40 min后,PR%、PR精子总数、PR+NP%和活动精子总数显著下降,差异均有统计学意义(P=0.000、P=0.000、P=0.000和P=0.000)。结论精液优化处理后的孵育时间与精液质量参数均呈负相关,优化处理后精液孵育至授精时间应控制在40 min内。Objective To determine whether semen parameters were affected by the time interval from optimizing preparation of sperm to intrauterine insemination(IUI)/in vitro fertilization(IVF)/intracytoplasmic sperm injection(ICSI).Methods 400 semen specimens from normozoospermia,oligozoospermia,asthenozoospermia and teratozoospermia men collected at our clinic.After density gradient centrifugation,samples were immediately kept in an incubator.The concentration,motility and progressive motility of sperm were respectively recorded after 20,40,80 and 120 minutes.Results The values of progressive sperm motility,progressive motile sperm count,total motility and total motile sperm count were significant decreased after 20 minutes in group asthenozoospermia(P=0.000,P=0.000,P=0.000 and P=0.000),they were also significant decreased after 40 minutes in group normozoospermia,group oligozoospermia and group teratozoospermia(P=0.000,P=0.000,P=0.000 and P=0.000).Conclusion Sperm parameters were negatively affected by prolonged incubation time.The maximum limit time of the interval from the onset of postwash sperm incubation to IUI/IVF/ICSI should be controlled within 40 minutes.
