基于慢性乙型肝炎的中成药应用现状探讨DRGs付费制度下中成药存在的问题及对策

摘　　要：目的通过分析慢性乙型肝炎中成药使用现状,探讨DRGs付费制度下临床应用中成药面临的问题及对策。方法检索CNKI中文期刊全文数据库有关慢性乙型肝炎的文献(检索年限为2012年1月至2016年12月),筛选出临床实证研究文献。结果慢性乙型肝炎的治疗以中西医结合为主,中成药治疗慢性乙型肝炎疗效确切且符合药物经济学原理,但存在中医辨证论治与DRGs的诊断不统一、DRGs付费制度缺乏对中医本身特色的补偿机制的问题。结论建议统一中医辨证论治与DRGs的诊断,并建立起对中医本身特色的补偿机制,将中成药纳入DRGs付费制度。Objective To explore the problems and countermeasures of the clinical application of Chinese patent medicine under the payment system of DRGs.Methods To search the literatures of CNKI in Chinese periodical database on Chronic hepatitis B(CHB)(from January 2012 to December 2016).Results The treatment of CHB was given priority to with combination of Chinese and western medicine,proprietary Chinese medicine treatment of CHB efficacy and conform to the principle of pharmacoeconomics,but the existence of traditional Chinese medicine syndrome differentiation and diagnosis of DRGs is not unified,lack of DRGs payment system problems of the compensation mechanism of TCM itself characteristics.Conclusion It is suggested to unify the diagnosis of TCM syndrome differentiation and DRGs,and to build up the compensation mechanism of TCM itself,and integrate the Chinese medicine into the payment system of DRGs.
