不同麻醉方式用于腹膜透析置管术的效果对比研究

摘　　要：目的研究不同麻醉方式用于腹膜透析置管术的效果。方法 40例行腹膜透析置管术的肾功能衰竭患者,采用随机数表法分为对照组与研究组,各20例。对照组实施气管插管全身麻醉(全麻),研究组采取超声引导下腹横筋膜平面神经阻滞麻醉。比较两组患者不同时间点心率与平均动脉压的变化,以及与术后麻醉相关不良反应。结果研究组注射麻醉药物后5、15 min时心率与平均动脉压高于对照组,差异具有统计学意义(P<0.05)。研究组不良反应发生率为15.0%,低于对照组的60.0%,差异具有统计学意义(P<0.05)。结论相较于气管内插管全麻,超声引导下腹横筋膜平面神经阻滞麻醉在腹膜透析置管术中的应用效果与安全性更佳,适于临床推广。Objective To research the effect of different anesthetic methods for peritoneal dialysis catheterization.Methods A total of 40 patients with renal failure who underwent peritoneal dialysis catheterization were divided into control group and research group according to the random number table method,20 cases in each group.The control group received general anesthesia with endotracheal intubation(general anesthesia).The research group received ultrasound guided transverse fascia plane nerve block anesthesia.The changes of heart rate and mean arterial pressure at different times,and the adverse reactions associated with postoperative anesthesia between the two groups were compared.Results The heart rate and mean arterial pressure of the research group at 5,15 min after injection of narcotic drugs were significantly higher than those of the control group,the differences were statistically significant(P<0.05).The incidence of adverse reactions in the research group was 15.0%,which was significantly lower than 60.0%in the control group,the difference was statistically significant(P<0.05).Conclusion Compared with the general anesthesia of endotracheal intubation,ultrasound guided transverse fascia plane nerve block anesthesia is more effective and safer in peritoneal dialysis catheterization,which is suitable for clinical application.
