摘 要:目的冠状动脉介入术中的设备参数设定,患者一般情况及术中医师的策略都会影响X线的放射剂量。评估造影时降低帧率(f/s)和曝光剂量时X线的总剂量及图像质量。方法纳入215例患者,随机分成对照组和剂量管理组(DMP组)2组。对照组在冠状动脉(冠脉)造影中应用标准帧率和标准曝光剂量(15 f/s和0.200μGy/fr);DMP组在冠脉造影中降低帧率和曝光剂量(7.5 f/s和0.120μGy/fr)。主要终点为总放射线剂量(AK;m Gy),次要终点为放射时间、对比剂用量、手术并发症发生率和图像质量。两组患者的基线资料相匹配。结果与对照组相比,DMP组的X线总放射剂量明显减低(181.4 vs.352.7 m Gy;P<0.01)。两组间的次要终点无统计学差异。DMP组在图像质量评分上不劣于对照组(8.79±1.05 vs.8.91±0.91;P=0.37)。结论冠脉造影中应用低帧率和曝光剂量模式,在不改变图像质量的情况下,明显降低了总放射线剂量。Objective To review the total dose of X-ray and imaging quality when reducing frame rate and exposure dose during the period of coronary angiography because equipment parameter determination,patients’general condition and doctors’strategy can influence X-ray dose during PCI procedure.Methods The patients(n=215)were chosen and divided randomly into control group and dose management protocol group(DMP group).The standard frame rate(15 f/s)and standard exposure dose(0.200μGy/fr)were used in control group.The reduced frame rate(7.5 f/s)and reduced exposure dose(0.120μGy/fr)were used in DMP group.The primary endpoint was the total radiation dose,and secondary endpoints were radiation time,dosage of contrast agent,incidence of complications and image quality.Results The total X-ray radiation dose was decreased significantly in DMP group compared with control group(181.4 vs.352.7 mGy;P<0.01).There was no statistical difference in secondary endpoints between 2 groups.The scores of image quality in DMP group were not inferior to those in control group(8.79±1.05 vs.8.91±0.91;P=0.37).Conclusion The application of lower frame rate and lower exposure dose can significantly reduce the total radiation dose without altering image quality.
