摘　　要：目的探讨不同浓度肝素封管法对血液透析患者深静脉留置导管的影响及护理分析。方法选取80例行深静脉置管的血液透析患者作为研究对象,采用随机数字表法分成对照组与研究组。对照组仅用肝素钠浓度为10 U/ml的0.9%Na Cl溶液进行封管,研究组患者则采用纯肝素钠溶液进行封管。观察两组封管效果以及凝血效果,其中封管情况记录包括患者出现导管堵塞、血流量不足、置管处渗血的发生率以及导管留置时间等指标;凝血情况包括纤维蛋白原(Fib)、凝血酶原时间(PT)、凝血酶时间(TT)、活化部分凝血酶时间(APTT)等指标,并对封管情况与凝血情况进行对比分析。结果研究组患者出现不良事件发生率显著低于对照组患者,其封管效果显著优于对照组,对比差异具有统计学意义(P<0.05)。研究组患者与对照组患者在凝血指标上对比差异不具有统计学意义(P>0.05)。结论采用纯肝素钠进行封管有不错的封管效果及抗凝效果,可以有效提高导管使用时间。Objective To explore the effect of different concentrations of heparin sealing method on venous indwelling catheter in hemodialysis patientsand nursing analysis.Methods 80 hemodialysis patients with deep vein catheter were included in this study.The patients were divided into the control group and the study group.The catheters in the control group were sealed with 10 U/ml of heparin sodium.The catheters in the study group were sealed with pure heparin sodium solution.The effect of two groups were compared including catheter blockage,insufficient blood flow,incidence of bleeding at catheter placement and retention time of the catheter.Coagulation index were recorded including Fibrinogen(Fib),prothrombin time(PT),thrombin time(TT),activated partial thromboplastin time(APTT).Results The sealing effects in the study group were better than the control and the incidence of adverse events in the study group was significantly lower than the control(P<0.05).There was no significant effect in coagulation system between the two group(P>0.05).Conclusion The use of pure heparin sodium method has better sealing effect and anticoagulation effect,which can effectively improve the duration time of the catheter.
