微创前方入路半髋关节置换术治疗80岁以上高龄股骨颈骨折患者的临床疗效观察

摘　　要：目的探讨微创前方入路半髋关节置换术治疗80岁以上高龄股骨颈骨折患者的临床疗效。方法选取2014年3月至2017年12月该院收治的80岁以上股骨颈骨折患者80例作为对象对象,按不同治疗方式随机分为观察组和对照组,每组40例。对照组行微创外侧入路半髋关节置换手术治疗,观察组行微创前方入路半髋关节置换术治疗。观察两组患者手术切口长度、术中出血量、手术时间及卧床时间的差异,分析并发症发生情况。结果观察组患者手术切口长度[(10.21±1.28)cm]、术中出血量[(139.4±31.9)mL]、手术时间[(87.73±19.38)min]及卧床时间[(2.5±1.4)d]均明显优于对照组[分别为(15.33±2.32)cm、(251.2±71.0)mL、(98.26±23.1)min、(4.6±2.3)d],差异均有统计学意义(P<0.05)。两组患者术后均出现再次意外骨折1例,经治疗后好转,观察组患者总不良反应发生率[7.5%(3/40)]明显低于对照组[20.0%(8/40)],差异有统计学意义(P<0.05)。结论微创前方入路半髋关节置换术对高龄股骨颈骨折患者的治疗创伤小,且并发症发生率较低。Objective To explore the clinical effect of minimally invasive anterior approach hemiarthroplasty in the treatment of femoral neck fractures over the age of 80 years.Methods Choose our hospital 80 patients over 80 years old patients with femoral neck fractures as the research object,randomly divided into two groups with 40 cases in each group,the control group underwent minimally invasive lateral approach of hip replacement surgery,the observation group underwent minimally invasive anterior hip replacement treatment,observe two groups of patients with incision length,the amount of bleeding,operation time and bed time difference and analysis of complications.Results The length of incision,the amount of bleeding in the operation,the time of operation and the bed time of the observation group were significantly better than those of the control group(P<0.05).1 cases of recurrent fracture appeared in two groups after operation.After treatment,the incidence of total adverse reactions in the control group was significantly higher than that in the observation group,with a significant difference(P<0.05).Conclusion The minimally invasive anterior approach for hemiarthroplasty is less invasive for the elderly patients with femoral neck fractures,and the incidence of complications is significantly lower.
